摘要
目的:探究血清淀粉样蛋白A(serum amyloid A,SAA)、中性粒细胞/淋巴细胞比值(neutrophil-to-lymphocyte ratio,NLR)及降钙素原(procalcitonin,PCT)联合检测对COPD合并肺部感染预后的预测效果。方法:选取南昌市洪都中医院2021年3月—2024年4月接受治疗的COPD合并肺部感染患者102例。入院后30 d,根据患者预后情况将其分为试验组[预后不良(死亡),n=25]和对照组[预后良好(存活),n=77]。比较两组入院时SAA、NLR及PCT水平,绘制ROC曲线分析SAA、NLR、PCT及三者联合检测对预后情况的预测效果。结果:试验组入院时SAA、NLR及PCT水平高于对照组(P<0.05);SAA、NLR、PCT三者联合诊断的AUC为0.958[95%CI(0.899,0.988)],敏感度为96.00%,特异度为94.80%。结论:SAA、NLR及PCT三者联合检测对COPD合并肺部感染患者预后不良具有较好的预测价值,敏感度和特异度较高。
Objective:To explore the predictive effect of combined detection of serum serum amyloid A(SAA),neutrophil-to-lymphocyte ratio(NLR),and procalcitonin(PCT)in patients with COPD complicated with pulmonary infection.Method:A total of 102 patients with COPD complicated with pulmonary infection who received treatment at Nanchang Hongdu Hospital of TCM from March 2021 to April 2024 were selected.Thirty days after admission,according to the prognosis of the patients,they were divided into the experimental group[poor prognosis(death),n=25]and the control group[good prognosis(survivac),n=77].The levels of SAA,NLR and PCT at admission were compared between the two groups,and the ROC curve was drawn to analyze the predictive effect of SAA,NLR,PCT and the combined detection of the three on prognosis.Result:The levels of SAA,NLR and PCT in the experimental group at admission were higher than those in the control group(P<0.05).The AUC of the combined diagnosis of SAA,NLR and PCT was 0.958[95%CI(0.899,0.988)],with a sensitivity of 96.00%and a specificity of 94.80%.Conclusion:The combined detection of SAA,NLR and PCT has a good predictive value for the poor prognosis of patients with COPD complicated with pulmonary infection,with high sensitivity and specificity.
