摘要
目的:探究责任制护理模式对呼吸衰竭患者心理状态及肺功能的影响。方法:选取2022年2月—2025年2月上饶市人民医院呼吸与危重症医学科收治的98例呼吸衰竭患者,按照随机数字表法分为常规护理组和责任制护理组,各49例;中途因病情恶化、转院、不配合研究等因素,12例患者脱落,最终86例患者纳入此次前瞻性研究,其中常规护理组42例,责任制护理组44例。常规护理组予以呼吸衰竭的标准护理,责任制护理组予以责任制护理模式。观察两组的症状缓解时间、住院时间、肺功能指标及心理状态评分。结果:责任制护理组住院时间短于常规护理组,咳嗽缓解时间、气促缓解时间均早于常规护理组(P<0.05)。干预2周后,责任制护理组FEV_(1)、FVC及呼气峰流量(PEF)水平高于常规护理组,SAS、SDS评分低于常规护理组(P<0.05)。结论:责任制护理模式可促进呼吸衰竭患者的康复,有利于气促、咳嗽等症状的缓解,可改善患者的肺功能和心理状态。
Objective:To explore the effect of responsibility nursing mode on psychological state and lung function of patients with respiratory failure.Method:A total of 98 patients with respiratory failure who were admited to the Department of Respiratory and Critical Care Medicine of Shangrao People's Hospital from February 2022 to February 2025 were selected and divided into routine nursing group and responsibility nursing group according to the random number table method,with 49 cases in each group.Midway due to the deterioration of the disease,transfer,do not cooperate with the study and other factors,12 patients fell off,and finally 86 patients were included in the prospective study,including 42 patients in the routine nursing group and 44 patients in the responsibility nursing group.The routine nursing group was given standard nursing for respiratory failure,and the responsibility nursing group was given responsibility nursing mode.The symptom relief time,hospitalization time,lung function index and psychological state score of the two groups were observed.Result:The hospitalization time of the responsibility system nursing group was shorter than that of the routine nursing group,the cough relief time and shortness of breath relief time of the responsibility nursing group were all earlier than those of the routine nursing group(P<0.05).Two weeks after the intervention,the levels of FEV_(1),FVC and peak expiratory flow(PEF)in the responsibility nursing group were higher than those in the routine nursing group,the scores of SAS and SDS were lower than those in the routine nursing group(P<0.05).Conclusion:The responsibility nursing mode can promote the rehabilitation of patients with respiratory failure,which is conducive to the relief of symptoms such as shortness of breath and cough,and can improve the lung function and psychological state of patients.
