美式整脊治疗腰椎节段性失稳的研究概况

摘要 本文采用文献资料法等研究方法,基于PubMed及中国知网(CNKI)数据库1998—2025年的相关文献,对美式整脊技术在腰椎节段性失稳治疗中的应用机制及临床疗效进行系统综述。研究发现:美式整脊技术通过矫正关节错位、调节脊柱生物力学平衡及激活核心稳定肌群等途径,改善腰椎节段稳定性;临床证据显示,该技术可显著缓解患者疼痛症状、改善腰椎功能障碍。我国传统正骨手法与美式整脊技术在治疗理念、操作范式上存在差异,但二者跨学科融合已显现出协同治疗效应。建议未来开展多中心、大样本临床研究以优化治疗方案,同时积极推进人工智能辅助诊断、机器人辅助手法操作等智能技术在脊柱康复领域的应用,进而提升腰椎节段性失稳的诊疗精准度与康复预后效果。 This study systematically reviews the therapeutic mechanisms and clinical efficacy of American Chiropractic Manipulative Treatment(CMT)in addressing lumbar segmental spinal instability through a comprehensive analysis of literature from PubMed and the China National Knowledge Infrastructure(CNKI)database(19982025).Findings suggest that CMT enhances lumbar segmental spinal stability via three key pathways:correcting vertebral misalignments,restoring spinal biomechanical equilibrium,and optimizing activation patterns of core stabilizing musculature.Clinically,CMT demonstrates significant efficacy in pain reduction and functional improvement.While fundamental differences exist between CMT and traditional Chinese osteopathic techniques regarding theoretical frameworks and operational protocols,emerging interdisciplinary integration strategies exhibit promising synergistic therapeutic effects.Future recommendations include conducting multicenter,large-sample clinical studies to optimize treatment protocols and advancing the application of intelligent technologies—such as AI assisted diagnosis and robot assisted manipulative techniques—in spinal rehabilitation.These advancements aim to enhance diagnostic and therapeutic precision and improve rehabilitation outcomes for lumbar segmental spinal instability.
