摘要目的:观察甲苯磺酸瑞马唑仑在不同年龄老年胃肠手术患者中的应用效果。方法:选取苏州市立医院2020年6月—2023年8月收治的160例行胃肠手术的老年患者,分为老年过渡期组、老年早期组,每组采取随机数字表法分成R亚组和D亚组,各40例。R亚组应用甲苯磺酸瑞马唑仑麻醉,D亚组应用以右美托咪定为首的常规全身麻醉方案。对比各组麻醉效果。结果:气管插管即刻,各组D亚组平均动脉压、心率低于R亚组(P<0.05);手术结束,各组D亚组心率低于R亚组(P<0.05)。各组R亚组苏醒时间、拔管时间短于D亚组(P<0.001)。各组D亚组心动过缓发生率高于R亚组(P<0.05)。老年早期组R亚组术后认知功能障碍发生率低于D亚组(P=0.013)。结论:甲苯磺酸瑞马唑仑在老年患者胃肠手术中的应用效果更高,具有循环抑制更轻、苏醒时间和拔管时间更短、心动过缓发生率更低的优势,且在老年早期患者中具有术后认知功能障碍发生率更低的优势。 Objective:To observe the application effect of remimazolam tosilate in elderly patients undergoing gastrointestinal surgery at different age groups.Methods:A total of 160 elderly patients who underwent gastrointestinal surgery in Suzhou Municipal Hospital from June 2020 to August 2023 were selected and divided into the transitional elderly group and the early elderly group.Each group was divided into R subgroup and D subgroup by using random number table method,with 40 cases in each group.In R subgroup,remimazolam tosilate was used for anesthesia,and in D subgroup,dexmedetomidine-predominant conventional general anesthesia regimen was used.The anesthetic effects in each group were compared.Results:Immediately after tracheal intubation,the mean arterial pressure and heart rate in D subgroup were lower than those in R subgroup(P<0.05).At the end of surgery,the heart rate in D subgroup was lower than that in R subgroup(P<0.05).The recovery time and extubation time in R subgroup were shorter than those in D subgroup(P<0.001).The incidence of bradycardia in D subgroup was higher than that in R subgroup(P<0.05).The incidence of postoperative cognitive dysfunction in R subgroup was lower than that in D subgroup within the early elderly group(P=0.013).Conclusion:Remimazolam tosilate has a higher application effect in elderly patients undergoing gastrointestinal surgery,with the advantages of milder circulatory depression,shorter recovery time and extubation time,lower incidence of bradycardia,and lower incidence of postoperative cognitive dysfunction in early elderly patients.
