摘要
目的:探究B超引导下股神经阻滞联合氢吗啡酮在老年全膝关节置换术后镇痛中的应用效果。方法:选取2024年5月—2025年5月于合肥市播州区人民医院进行全膝关节置换术的80例老年患者,随机分为对照组、观察组,各40例。对照组术后采取常规自控镇痛,观察组在对照组基础上采取B超引导下股神经阻滞联合氢吗啡酮。比较两组镇痛情况、应激指标和不良反应发生率。结果:相比于对照组,观察组术后2、24、48 h视觉模拟评分法评分更低(P<0.05);术后2 h,观察组心率、平均动脉压水平比对照组低(P<0.05);观察组不良反应发生率比对照组低(P=0.043)。结论:B超引导下股神经阻滞联合氢吗啡酮有利于缓解老年全膝关节置换术患者术后疼痛,稳定应激反应,减少不良反应。
Objective:To explore the application effect of B-mode ultrasound-guided femoral nerve block combined with hydromorphone in postoperative analgesia of elderly patients undergoing total knee arthroplasty.Methods:A total of 80 elderly patients who underwent total knee arthroplasty at the People's Hospital of Zunyi City Bozhou District from May 2024 to May 2025 were selected.They were randomly divided into control group and observation group,with 40 patients in each group.Postoperatively,the control group received routine self-controlled analgesia,while the observation group received femoral nerve block under B-mode ultrasound guidance combined with hydromorphone on the basis of control group.The analgesic effects,stress indexes and incidence of adverse reactions were compared between the two groups.Results:The visual analogue scale scores at 2 h,24 h and 48 h after surgery in the observation group were lower than those in the control group(P<0.05).At 2 h after surgery,the heart rate and mean arterial pressure in the observation group were lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P=0.043).Conclusion:B-mode ultrasound-guided femoral nerve block combined with hydromorphone is beneficial for relieving postoperative pain,stabilizing stress response and reducing adverse reactions in elderly patients undergoing total knee arthroplasty.
