应激系统理论下的分段式盆底肌锻炼在机器人辅助腹腔镜子宫切除术中的应用效果

摘要 目的:探究应激系统理论下的分段式盆底肌锻炼对机器人辅助腹腔镜子宫切除术患者膀胱功能及女性性功能量表(FSFI)评分的影响。方法:选取2022年5月—2025年3月于西北妇女儿童医院行机器人辅助腹腔镜子宫切除术的81例患者,根据术后干预模式的差异将其分为对照组(采取常规干预,n=40)与观察组(采取应激系统理论下的分段式盆底肌锻炼,n=41)。比较两组患者的围手术期指标、盆底肌电值、膀胱功能、性功能以及创伤后应激障碍情况。结果:与对照组相比,观察组首次下床时间、肛门排气时间、首次进食时间及住院时间均更短(P<0.05)。术后8周,两组患者的快肌收缩最大值、10 s慢肌收缩平均值、60 s慢肌收缩平均值、FSFI各维度评分及总分均呈升高趋势,且观察组的升高趋势更为显著(P<0.05)。术后8周,两组患者的创伤后应激障碍检查表(PCL-C)评分、排尿功能障碍量表(MHU)各维度评分及总分均有所降低,且与对照组相比,观察组以上评分均更低(P<0.05)。结论:应激系统理论下的分段式盆底肌锻炼可加快机器人辅助腹腔镜子宫切除术患者康复进展,提高盆底功能、膀胱功能、性功能,缓解创伤后应激障碍。 Objective:To explore the effect of segmented pelvic floor muscle exercises based on the stress theory on bladder function and female sexual function index(FSFI)scores in patients undergoing robot-assisted laparoscopic hysterectomy.Methods:81 patients who underwent robot-assisted laparoscopic hysterectomy at Northwest Women’s and Children’s Hospital from May 2022 to March 2025 were enrolled.They were divided into the control group(n=40,receiving routine intervention)and the observation group(n=40,receiving segmented pelvic floor muscle exercises based on the stress theory)based on postoperative intervention strategies.Perioperative indicators,pelvic floor electromyography values,bladder function,sexual function,and post-traumatic stress disorder(PTSD)were compared between the two groups.Results:Compared to the control group,the observation group had significantly shorter times to first out-of-bed activity,first flatus,first oral intake,and hospital stay(P<0.05).At 8 weeks postoperatively,both groups showed increased trends in maximum fast-twitch muscle contraction,average 10 s slow-twitch muscle contraction,average 60 s slow-twitch muscle contraction,FSFI scores,and total scores,with a more significant increase in the observation group(P<0.05).At 8 weeks postoperatively,PTSD checklist-civilian version(PCL-C)scores,MHU scores,and total scores were decreased in the two groups,with significantly lower scores in the observation g roup(P<0.05).Conclusion:Segmented pelvic floor muscle exercises under the stress theory can accelerate postoperative recovery,improve pelvic floor function,bladder function,sexual function,and alleviate PTSD in patients undergoing robot-assisted laparoscopic hysterectomy.
