摘要
目的:探讨上肢机器人任务导向康复训练对脑卒中偏瘫患者上肢功能及生活质量的影响。方法:选取2023年6月—2024年12月于安徽医科大学第一附属医院接受治疗的144例脑卒中偏瘫患者,随机分为研究组(常规治疗+上肢机器人任务导向康复训练,72例)和对照组(常规治疗+传统任务导向训练,72例)。比较两组患者的上肢功能、关节活动度、肌力、日常生活能力、生活质量及安全性。结果:干预后各时点,两组患者Fugl-Meyer上肢运动功能评分量表(FMA-UE)评分均高于干预前,且研究组高于对照组(P<0.05)。干预后,两组患者的关节活动体积、深度、高度、宽度均高于干预前,且研究组高于对照组(P<0.05)。干预后,两组患者在矢状轴、冠状轴方向的肌力均高于干预前,且研究组高于对照组,但在垂直轴方向的肌力低于干预前,且研究组低于对照组(P<0.05)。干预4周、8周后,两组患者的改良Barthel指数(MBI)、脑卒中患者生活质量疾病专用量表(SIS)评分高于干预前,且研究组高于对照组(P<0.05)。两组患者不良事件总发生率比较,差异无统计学意义(P>0.05)。结论:上肢机器人任务导向康复训练可改善脑卒中偏瘫患者的上肢功能、日常生活能力及生活质量,且安全性良好。
Objective:To investigate the effects of upper-limb robot-assisted task-oriented rehabilitation training on upper-limb function and quality of life in stroke patients with hemiplegia.Methods:A total of 144 stroke patients with hemiplegia treated at the First Affiliated Hospital of Anhui Medical University from June 2023 to December 2024 were selected.They were randomly divided into the study group(n=72,receiving conventional therapy combined with upper-limb robot-assisted task-oriented training)and the control group(n=72,receiving conventional therapy combined with traditional task-oriented training).Upper-limb function,joint range of motion,muscle strength,activities of daily living(ADL),quality of life,and safety were compared between the two groups.Results:At all timepoints after intervention,higher Fugl-Meyer assessment for upper extremity(FMA-UE)scores were observed in both groups,with the study group scoring significantly higher than the control group(P<0.05).After intervention,joint movement volume,depth,height,and width increased in the two groups compared to those before intervention,with greater improvements in the study group than the control group(P<0.05).Muscle strength in the sagittal and coronal axes increased after intervention in the two groups,with greater improvements in the study group than the control group;while vertical-axis strength decreased,and it was lower in the study group than the control group(P<0.05).At 4 and 8 weeks after intervention,both groups showed higher modified Barthel index(MBI)and stroke-specific quality of life scale(SIS)scores than those before intervention,with the study group scoring higher than the control group(P<0.05).No statistically significant difference was observed in the total incidence of adverse events between the two groups(P>0.05).Conclusion:Upper-limb robot-assisted task-oriented rehabilitation training can improve upper-limb function,ADL,and quality of life in hemiplegic stroke patients with good safety.
