摘要
目的:探讨机器人辅助单孔胸腔镜手术(URATS)和单孔电视胸腔镜手术(UVATS)在早期肺癌中的围手术期安全性及可行性。方法:回顾性选取2023年4月13日—2024年4月11日于南通大学附属医院胸外科由同一团队行手术治疗的早期肺癌患者为研究对象,按手术方式分为URATS组与UVATS组。对两组患者进行倾向性评分匹配,比较匹配后两组患者在手术安全性及早期疗效方面的差异。结果:共成功匹配42对患者。两组在年龄、性别、手术切除部位、切除范围、TNM分期及病理类型等方面差异均无统计学意义(P>0.05)。与UVATS组相比,URATS组手术时间较长,但术中淋巴结清扫数目更多,出血量更少,术后住院时间也更短,差异均具有统计学意义(P<0.05)。结论:URATS用于治疗早期肺癌安全可靠,在增加淋巴结清扫数目、减少术中出血以及缩短术后住院时间方面具有明显优势,值得在已常规开展单孔电视胸腔镜手术及机器人辅助手术的医疗机构中推广应用。
Objective:To compare the perioperative safety and feasibility of uniportal robot-assisted thoracoscopic surgery(URATS)and uniportal video-assisted thoracoscopic surgery(UVATS)in the treatment of early-stage lung cancer.Methods:A retrospective analysis was conducted on patients with early-stage lung cancer who underwent surgery performed by the same team at the Department of Thoracic Surgery of the Affiliated Hospital of Nantong University from April 13,2023 to April 11,2024.The patients were divided into the URATS group and the UVATS group based on the surgical approach.Propensity score matching was applied,and differences in surgical safety and early outcomes were compared between the two groups after matching.Results:A total of 42 matched pairs were included.No significant differences were observed between the two groups in terms of age,sex,surgical resection site,extent of resection,TNM stage,or pathological type(P>0.05).Compared with the UVATS group,the URATS group had a longer operative time but a higher number of lymph nodes dissected,less intraoperative blood loss,and a shorter postoperative length of hospital stay,with all differences being statistically significant(P<0.05).Conclusion:URATS is safe and reliable for the treatment of early-stage lung cancer.It shows significant advantages in increasing the number of lymph nodes dissected,reducing intraoperative blood loss,and shortening length of hospital stay,making it worthy of promotion in medical institutions that routinely perform uniportal video-assisted thoracoscopic surgery and robot-assisted surgeries.
