摘要
目的:探究数字化设计结合3D打印个性化经皮导板在肩关节周围穿刺中的应用效果及对术后功能恢复的影响。方法:选取2021年9月—2024年9月于本院接受肩关节周围穿刺治疗的106例病例进行前瞻性临床研究。采用区组随机化方法将受试者均衡分为研究组(n=53)与对照组(n=53)。对照组实施传统超声引导穿刺,研究组实施数字化设计结合3D打印个性化经皮导板辅助穿刺,均随访6个月。比较两组手术相关指标、疼痛程度(NRS评分)、肩关节功能(CMS评分)、肩关节活动度及并发症发生情况之间的差异。结果:研究组穿刺误差、穿刺时间、透视次数均低于对照组(P<0.05)。相较于治疗前,两组患者治疗后4周、治疗后8周及治疗后12周NRS评分均降低(P<0.05),且研究组治疗后8周NRS评分低于对照组(P<0.05)。相较于治疗前,两组患者治疗后4周、治疗后12周及治疗后24周CMS评分均升高(P<0.05),且研究组高于对照组(P<0.05)。与治疗前相比,两组患者治疗后24周肩关节活动度均升高(P<0.05),且研究组高于对照组(P<0.05)。研究组并发症总发生率低于对照组(P<0.05)。结论:数字化设计结合3D打印个性化经皮导板应用于肩关节周围穿刺可有效缓解术后疼痛程度,促进术后肩关节功能恢复,降低术后并发症发生率。
Objective:To explore the application effect of digital design combined with 3D-printed personalized percutaneous guide in shoulder arthrocentesis and its effect on postoperative functional recovery.Methods:A prospective clinical study was conducted on 106 patients who underwent shoulder arthrocentesis at our hospital from September 2021 to September 2024.They were divided into the study group(n=53)and the control group(n=53)using the block randomization method.The control group received conventional ultrasound-guided puncture,while the study group underwent puncture assisted by digital design combined with a 3D-printed personalized percutaneous guide.All patients were followed up for 6 months.Differences in surgical indicators,pain level[numerical rating scale(NRS)score],shoulder joint function[Constant-Murley score,(CMS)],shoulder joint range of motion,and complication rates were compared between the two groups.Results:The puncture error,puncture time,and number of fluoroscopies in the study group were lower than those in the control group(P<0.05).Compared with those before treatment,NRS scores at 4 weeks,8 weeks,and 12 weeks after treatment decreased in both groups(P<0.05),and the NRS scores in the study group at 8 weeks after treatment were lower than those in the control group(P<0.05).Compared with those before treatment,CMS scores at 4 weeks,12 weeks,and 24 weeks after treatment increased in both groups(P<0.05),and CMS scores in the study group were higher than those in the control group(P<0.05).Compared with that before treatment,shoulder joint range of motion at 24 weeks after treatment increased in the two groups(P<0.05),and the study group showed a greater range of motion than the control group(P<0.05).The total complication rate in the study group was lower than that in the control group(P<0.05).Conclusion:The application of digital design combined with 3D-printed personalized percutaneous guide in shoulder arthrocentesis can effectively alleviate postoperative pain,promote postoperative shoulder joint functional recovery,and reduce the incidence of postoperative complications.
