摘要
目的检索、评价和整合国内外先天性心脏病患者过渡到成年人医疗的相关证据,为先天性心脏病患者过渡到成年人医疗提供依据。方法系统检索BMJ最佳临床实践等相关网站和国内外数据库中关于先天性心脏病患者过渡到成年人医疗的证据,检索类型包括临床决策、指南、专家共识、系统评价、随机对照试验、证据总结,检索时间为建库至2025年2月16日。由2名研究者对纳入文献进行质量评价,并对符合质量标准的文献进行证据提取。结果纳入的15篇文献中,包括2篇临床决策、4篇指南、6篇专家共识、2篇证据总结、1篇随机对照试验,涵盖了过渡原则、过渡准备、过渡期个体化照护、向成年人医疗转移4个方面,共汇总38条最佳证据。结论该研究为医护人员整合了先天性心脏病患者过渡到成年人医疗的循证依据。临床实践者可基于临床情境,帮助先天性心脏病患者从12岁左右准备过渡,过程中关注神经发育障碍的患者,进行个体化护理,转移时医护多方参与、妥善交接,助力患者向成年人医疗转移。
Objective To retrieve,evaluate and integrate the relevant evidence of the transition from congenital heart disease patients to adult medical care at home and abroad,and provide a basis for the transition from congenital heart disease patients to adult medical care.Methods The evidence of the transition from congenital heart disease patients to adult medical care was systematically searched in the related websites and databases at home and abroad such as BMJ best clinical practice,etc.The search types included clinical decision-making,guidelines,expert consensus,systematic evaluation,evidence summary and randomized controlled trials.The search time was from the establishment of the database to February 16,2025.Two researchers evaluated the quality of the included literature and extracted evidence from the literature that met the quality standards.Results Among the 15 pieces of literature included,there were 2 clinical decision-making,4 guidelines,6 expert consensus,2 evidence summaries,and 1 randomized controlled trial,covering 4 aspects of transition principles,transition preparation,individualized care during transition,and medical transfer to adults.A total of 38 best evidences were summarized.Conclusions This study integrates the evidence-based basis for medical staff to transition from congenital heart disease patients to adult medical care.Based on the clinical situation,clinical practitioners can help the transition of adolescents with congenital heart disease around the age of 12,pay attention to patients with neuro-developmental disorders in the process,carry out individualized nursing,and participate in the proper handover of doctors and nurses during the transfer,so as to help patients transfer to adults.
