摘要
目的评估非洲工程项目施工营地建立的蚊媒防控“三道防线”体系在降低蚊虫密度及疟疾发病率方面的有效性,并优化该体系。方法在乍得4个已全面实施该体系的营地,于实施后的连续3个疟疾流行季节,监测居住区周边成蚊和蚊幼虫密度,并统计中方工作人员的年疟疾发病率。分析蚊媒密度变化与发病率的相关性,据此完善“三道防线”体系并融入配套管理措施。结果“三道防线”体系及配套管理措施的实施,使营地成蚊和蚊幼虫密度显著降低(均P≤0.01),下降率分别为88.79%和96.49%。该低密度状态在3年监测期内保持稳定(P≥0.05)。工作人员疟疾发病率与成蚊密度(r=0.838,P≤0.01)及蚊幼虫密度(r=0.851,P≤0.01)呈显著正相关。体系实施后,发病率降低了80.38%。结论在非洲工程项目营地建立并实施的蚊媒综合防控“三道防线”体系,结合配套管理措施,能有效、显著且持续地降低营地成蚊和蚊幼虫密度,从而显著降低中方工作人员的疟疾发病率。该体系效果稳定,具有重要的公共卫生价值,可在类似海外工程项目中推广应用。
Objective To evaluate the effectiveness of the"Three-Line Defense System"for mosquito vector control in the construction camps of African engineering projects in reducing mosquito density and malaria incidence,and to optimize this system.Methods In four camps in Chad where the system had been fully implemented,the densities of adult mosquitoes and larvae around residential areas were monitored over three consecutive malaria epidemic seasons following implementation.The annual malaria incidence among Chinese staff was also recorded.The correlation between changes in mosquito vector density and malaria incidence was analyzed,and the"Three-Line Defense System"was refined accordingly,incorporating supporting management measures.Results The implementation of the"Three-Line Defense System"and supporting management measures significantly reduced the densities of adult mosquitoes and larvae in the camps(both P≤0.01),with reduction rates of 88.79%and 96.49%,respectively.This low-density state remained stable throughout the three-year monitoring period(P≥0.05).The malaria incidence among staff showed a significant positive correlation with adult mosquito density(r=0.838,P≤0.01)and larvae density(r=0.851,P≤0.01).After system implementation,malaria incidence decreased by 80.38%.Conclusion The integrated"Three-Line Defense System"for mosquito vector control,combined with supporting management measures,the density of adult mosquitoes and larvae in African engineering project camps can be effectively,significantly and continuously reduced,thereby significantly lowering the malaria incidence among Chinese staff.This system demonstrates stable effectiveness and holds significant public health value,making it suitable for broader application in similar overseas engineering projects.
