摘要
目的探讨头戴式放大镜在全冠牙体预备中对口腔医生颈椎腰椎健康及预备体颈缘质量的影响。方法选取2022年12月至2024年12月于浙江中医药大学附属第二医院行全冠修复的205例牙体缺损患者,随机分为常规组(n=104,常规视野预备)和放大镜组(n=101,2.5倍头戴式放大镜下预备),比较两组医生的体位评分、颈腰椎不适评分及预备体颈缘质量。结果放大镜组医生的体位舒适度主观评分(7.96±1.12)分高于常规组(4.92±1.03)分,客观评分(14.03±1.24)分低于常规组(17.02±1.35)分;操作后颈肩部[(2.33±0.80)分vs(4.70±1.20)分]、腰背部[(2.03±0.70)分vs(4.33±1.10)分]不适评分均更低(P<0.05)。预备体颈缘质量方面,放大镜组硅橡胶阴模颈缘肩台宽度(0.64±0.20)mm小于常规组(0.87±0.10)mm(P<0.05);石膏代型颈缘合格率88.1%略高于常规组85.6%(P>0.05)。操作时长大于30min对颈腰椎不适影响显著(P<0.05)。结论头戴式放大镜可降低口腔医生的颈腰椎职业损伤风险,提高全冠牙体颈缘预备精度。
Objective To explore the impact of head-mounted loupes on stomatologists’cervical-lumbar health and prepared teeth’s marginal quality during full crown preparation.Methods 205 patients with tooth defects undergoing full crown restoration form December 2022 to December 2024 were grouped randomly.104 in conventional group received preparation under normal vision,and 101 in loupe group used 2.5×head-mounted loupes.Postural scores,cervical-lumbar discomfort,silicone impression cervical shoulder width,and gypsum die cervical marginal qualification were compared.Results The loupe group showed higher subjective postural scores(7.96±1.12)point vs(4.92±1.03)point and lower objective scores(14.03±1.24)point vs(17.02±1.35)point(P<0.05).Post-operation,the cervical-shoulder[(2.33±0.80)point vs(4.70±1.20)point]and lumbar-back discomfort scores[(2.03±0.70)point vs(4.33±1.10)point]were lower(P<0.05).The loupe group had smaller average cervical shoulder width(0.64±0.20)mm vs(0.87±0.10)mm(P<0.05),with slightly higher gypsum die qualification rate(88.1%vs 85.6%,P>0.05).Operation duration over 30 minutes significantly affected cervical-lumbar discomfort(P<0.05).Conclusion Head-mounted loupes can reduce the risk of occupational injuries to the cervical and lumbar spine of stomatologists and improve the precision of full crown preparation.
