摘要
目的探讨血清长链非编码RNA锌指蛋白反义链1(lncRNA ZFAS1)、长链非编码RNA同源盒基因转录反义RNA(lncRNA HOTAIR)对多囊卵巢综合征(PCOS)诊断及预后预测的价值。方法198例PCOS患者为观察组,另取同期体检健康育龄期女性198例为对照组。抽取两组空腹静脉血,采用实时荧光定量PCR法检测血清lncRNA ZFAS1、lncRNA HOTAIR。规范治疗3个月,超声复查结果判断预后;采用多因素Logistic回归分析法分析PCOS预后的影响因素,绘制受试者工作特征(ROC)曲线评估血清lncRNA ZFAS1、lncRNA HOTAIR对PCOS诊断及预后预测的价值。结果与对照组比较,观察组血清lncRNA ZFAS1、lncRNA HOTAIR相对表达量均高(P均<0.05)。198例PCOS患者中预后良好107例、预后不良91例;与预后不良者比较,预后良好者血清lncRNA ZFAS1、lncRNA HOTAIR相对表达量均低(P均<0.05)。多因素Logistic回归分析结果,血清lncRNA ZFAS1、lncRNA HOTAIR、抗缪勒管激素(AMH)水平升高是PCOS患者预后不良的危险因素(P均<0.05),促卵泡生成素(FSH)、内皮抑制素(ES)升高是PCOS患者预后不良的保护因素(P均<0.05)。ROC曲线结果,血清lncRNA ZFAS1、lncRNA HOTAIR单独及二者联合诊断PCOS时的曲线下面积(AUC)为0.712、0.750、0.813;血清lncRNA ZFAS1、lncRNA HOTAIR单独及二者联合预测PCOS预后不良的AUC为0.765、0.819、0.867。结论PCOS患者血清lncRNA ZFAS1、lncRNA HOTAIR水平升高。血清lncRNA ZFAS1、lncRNA HOTAIR水平对PCOS诊断及预后不良预测有一定价值。
Objective To investigate the predictive value of serum long non-coding RNA zinc finger protein antisense chain 1(lncRNA ZFAS1)and long non-coding RNA homologous box gene transcription antisense RNA(lncRNA HOTAIR)in the early diagnosis and prognosis of patients with polycystic ovary syndrome(PCOS).Methods Totally 198 PCOS patients(research group)and 198 healthy women of reproductive age who underwent physical examination during the same period(control group)were selected.Fasting venous blood was drawn from the subjects of the two groups.Real-time fluorescent quantitative PCR(qRT-PCR)was used to detect serum lncRNA ZFAS1 and lncRNA HOTAIR.Patients received the standardized treatment for 3 months,and the prognosis was determined based on the ultrasound re-examination results.Multivariate Logistic regression was applied to analyze the factors affecting the prognosis of PCOS.Receiver operating characteristic(ROC)curve was used to evaluate the value of serum lncRNA ZFAS1 and lncRNA HOTAIR in the diagnosis and prognosis prediction of PCOS.Results Compared with the control group,the research group had higher relative expression levels of serum lncRNA ZFAS1 and lncRNA HOTAIR(both P<0.05).Among 198 patients with PCOS,there were 107 patients with good prognosis and 91 patients with poor prognosis.Compared with patients with poor prognosis,patients with good prognosis had lower relative expression levels of serum lncRNA ZFAS1 and lncRNA HOTAIR(both P<0.05).Multivariate Logistic regression showed that elevated levels of serum lncRNA ZFAS1,lncRNA HOTAIR and AMH were risk factors affecting the prognosis of PCOS patients(all P<0.05),and elevated levels of FSH and ES were protective factors affecting the prognosis of PCOS patients(both P<0.05).ROC curve showed that the area under the curve(AUC)of serum lncRNA ZFAS1,lncRNA HOTAIR alone and in combination for the diagnosis of PCOS were 0.712,0.750,and 0.813,respectively.The AUC for predicting poor prognosis of PCOS using serum lncRNA ZFAS1 and lncRNA HOTAIR alone and in combination were 0.765,0.819,and 0.867,respectively.Conclusions The serum levels of lncRNA ZFAS1 and lncRNA HOTAIR in patients with PCOS are elevated.The levels of serum lncRNA ZFAS1 and lncRNA HOTAIR have certain value in the early diagnosis and poor prognosis evaluation of PCOS.
