摘要
目的分析中老年人群C反应蛋白-甘油三酯葡萄糖指数(CTI)与心脏代谢疾病(CMD)的关联。方法基于2015年中国健康与养老追踪调查(CHARLS)横断面数据进行研究,对选入变量采用方差膨胀因子(VIF)进行多重共线性检验。采用多因素Logistic回归评估CTI与CMD的关联,采用亚组分析评估CTI对不同亚组人群CMD发病影响的差异,采用限制性三次样条(RCS)分析非线性关系,采用受试者工作特征(ROC)曲线评估CTI对CMD的预测效能。结果数据共纳入7316例中老年人,其中CMD患者2397例(32.76%)。在调整所有混杂因素的Logistic回归模型中发现,CTI升高与CMD发病有关(OR=2.53,95%CI:2.24~2.86,P<0.001)。亚组分析显示,吸烟史对CTI与CMD的关联具有显著交互作用(P<0.05)。RCS分析显示,CTI与CMD发病存在显著非线性关联(P均<0.001)。ROC曲线分析显示,CTI对CMD有一定预测效能(曲线下面积为0.633,截断值为3.839)。结论CTI与中老年人群CMD发病存在关联,CTI检测可能有助于中老年人群CMD发病风险筛查。
Objective To analyze the association between the C-reactive protein-triglyceride-glucose index(CTI)and cardiometabolic diseases(CMD)among middle-aged and elderly Chinese adults.Methods This cross-sectional study was based on data from the 2015 China Health and Retirement Longitudinal Study(CHARLS).Multicollinearity among variables was examined using the variance inflation factor(VIF).Multivariate Logistic regression was performed to evaluate the association between CTI and CMD.Subgroup analysis was conducted to assess the effect of CTI on CMD in different subpopulations.Restricted cubic spline(RCS)models were used to explore nonlinear associations.Receiver operating characteristic(ROC)curve analysis was applied to assess the predictive efficiency of CTI for CMD.Results A total of 7,316 middle-aged and elderly participants were included,among whom 2,397(32.76%)had CMD.After adjusting for potential confounders,elevated CTI was significantly associated with CMD(OR=2.53,95%CI:2.24-2.86,P<0.001).Subgroup analysis revealed a significant interaction between smoking history and the CTI-CMD association(P<0.05).RCS analysis indicated a significant nonlinear relationship between CTI and CMD(both P<0.001).The ROC curve showed that CTI had moderate predictive performance for CMD(area under the curve=0.633;cut-off value3.839).Conclusion CTI is significantly associated with the presence of CMD in middle-aged and elderly Chinese adults and may serve as a useful indicator for CMD risk screening in this population.
