摘要
目的探讨内镜逆行胰胆管造影(ERCP)术后并发症的危险因素,并总结护理对策。方法选取2020年1月至2025年6月366例于南方医科大学南方医院行ERCP的胆胰疾病患者为研究对象,依据ERCP术后有无并发症分为并发症组(35例)和无并发症组(331例)。回顾性分析两组的临床资料,采用单因素及多因素Logistic回归分析患者ERCP术后并发症发生的危险因素。结果单因素分析结果显示,并发症组有ERCP史、ERCP时间≥30 min、胆管直径<5 mm、括约肌预切开、胰管插管≥2次和插管困难患者占比均高于无并发症组,差异具有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,ERCP史、ERCP时间≥30 min、胆管直径<5 mm、括约肌预切开、胰管插管≥2次、插管困难是ERCP术后并发症发生的独立危险因素(P<0.05)。结论对于有ERCP史、ERCP时间≥30 min、胆管直径<5 mm、括约肌预切开、胰管插管≥2次及插管困难的患者,临床应警惕及密切关注ERCP术后并发症的发生,并加强护理干预,以预防和减少严重不良事件。
Objective To explore the risk factors of complications after endoscopic retrograde cholangiopancreatography(ERCP)and summarize the nursing interventions.Methods A total of 366 patients with biliary and pancreatic diseases who underwent ERCP in Nanfang Hospital of Southern Medical University from January 2020 to June 2025 were selected as the research objects.According to the presence or absence of complications after ERCP,they were divided into complication group(35 cases)and non-complication group(331 cases).The clinical data of the two groups were retrospectively analyzed,and the risk factors of complications after ERCP were analyzed by univariate and multivariate Logistic regression analysis.Results Univariate analysis showed that the proportion of patients with ERCP history,ERCP time≥30 min,bile duct diameter<5 mm,sphincter precut,pancreatic duct intubation≥2 times and difficult intubation in the complication group were higher than that in the non-complication group,and the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that ERCP history,ERCP time≥30 min,bile duct diameter<5 mm,sphincter precut,pancreatic duct intubation≥2 times,and difficult intubation were independent risk factors for complications after ERCP(P<0.05).Conclusion For patients with ERCP history,ERCP time≥30 min,bile duct diameter<5 mm,sphincter precut,pancreatic duct intubation≥2 times and difficult intubation,clinical attention should be paid to the occurrence of complications after ERCP,and nursing interventions should be strengthened to prevent and reduce serious adverse eventsclinical attention.
