摘要
原发性支气管肺癌(简称肺癌)是世界各国发病率和死亡率较高的恶性肿瘤之一。李仝教授基于肺癌发病特点及其主要病机,概括总结出“癌毒伏邪”理论,强调肺癌多因正虚邪伏而发病,寒热痰瘀积聚而进展,有病程缠绵隐匿、病位广泛传变、病情危重难逆的特点。治疗强调扶正祛邪、分期辨治:早期“攻邪截毒”,清肃伏邪防传变;中期“攻补兼施”,破巢逐瘀涤痰兼扶正;晚期“固本培元”,补益元气。文章结合典型病例,将李仝教授对肺癌的诊疗思路与用药经验进行归纳总结,以飨同道。
Primary bronchogenic lung cancer(referred to as lung cancer for short)is one of the malignant tumors with relatively high incidence and mortality rates in countries around the world.Based on the characteristics of lung cancer and its main pathogenesis,professor LI Tong summarized the theory of'cancer toxin and lantent pathogen',emphasizing that lung cancer often occurs due to deficiency of the body's vital energy and the hiding of pathogenic factors,and progresses due to the accumulation of cold,heat,phlegm and blood stasis.It is characterized by a protracted and concealed course of the disease,extensive transmission and transformation of the lesion location,and a critical,difficult-to-reverse condition.The treatment emphasizes supporting the body's vital energy and eliminating pathogenic factors,as well as staged differentiation and treatment:in the early stage,'attacking pathogenic factors and intercepting toxins'should be carried out to clear and subdue pathogenic factors and prevent their transmission and transformation.In the middle stage,'a combination of attack and tonification'should be adopted to break the nest,remove blood stasis,eliminate phlegm and support the body's vital energy.In the late stage,'strengthening the foundation and nourishing the vital energy'should be carried out to replenish the vital energy.This article,based on typical cases,summarizes professor LI Tong's diagnosis and treatment ideas as well as his medication experience for lung cancer,to share with colleagues.
