摘要
目的探讨结直肠癌术后局部复发风险的影响因素。方法回顾性收集2022年1月—2024年1月泉州市第一医院收治的149例结直肠癌患者的临床资料,所有患者术后均随访1年,根据随访期间是否出现局部复发分为复发组31例和未复发组118例。比较两组患者的临床资料,筛选结直肠癌根治术后局部复发的影响因素,绘制受试者工作特征曲线分析预测价值。结果两组年龄、TNM分期、术中输血情况、病理切缘状态差异有统计学意义(P<0.05)。多因素logistic回归结果显示,年龄、TNM分期、病理切缘状态均为结直肠癌根治术后局部复发的影响因素(P<0.05)。各因素联合预测结直肠癌根治术后局部复发的AUC高于单独预测(P<0.05)。结论年龄、TNM分期、病理切缘状态均为结直肠癌术后局部复发的危险因素,三者联合预测结直肠癌术后局部复发价值较高。
Objective To explore the influencing factors of local recurrence risk after colorectal cancer surgery.Methods The clinical data of 149 colorectal cancer patients admitted to Quanzhou First Hospital from January 2022 to January 2024 were retrospectively selected.All patients were followed up for 1 year after surgery,and they were divided into a recurrence group(n=31)and a non-recurrence group(n=118)according to whether local recurrence occurred during the follow-up period.The clinical data of two groups of patients were compared to screen the influencing factors of local recurrence after radical resection of colorectal cancer,and draw a receiver operating characteristic(ROC)curve to analyze the predictive value.Results There were statistically significant differences in age,TNM staging,intraoperative blood transfusion,and pathological margin status between the two groups(P<0.05).Multivariate logistic regression results showed that age,TNM stage,and pathological margin status were risk factors for local recurrence after radical resection of colorectal cancer(P<0.05).The combined AUC of each factor for local recurrence after radical resection of colorectal cancer was higher than that of single factor prediction(P<0.05).Conclusion Age,TNM stage and pathological margin status are all risk factors for local recurrence after colorectal cancer surgery.The combination of the three has a relatively high predictive value for local recurrence after colorectal cancer surgery.
