摘要
目的探讨踝关节骨折伴胫腓损伤患者予以缝线环抱Nice结弹性固定的应用价值。方法选择2024年1—12月北京市门头沟区医院收治的踝关节骨折伴胫腓损伤患者120名,随机分为对照组和观察组各60名。对照组采用皮质骨螺钉固定,观察组采用缝线环抱Nice结弹性固定,对比两组优良率、关节形态结构及踝关节功能等指标。结果两组手术时间、骨折愈合时间、术中出血量、下床活动时间及住院时间差异无统计学意义(P>0.05);观察组完全负重时间短于对照组,差异有统计学意义(P<0.05)。观察组优良率高于对照组,差异有统计学意义(P<0.05)。术后3个月,两组下胫腓关节间隙、内侧关节间隙、踝穴宽度差水平均较术前降低,胫腓骨重叠较术前增加,但组间差异无统计学意义(P>0.05)。术后3个月,观察组踝关节功能评分和踝关节背伸度、踝关节跖屈度高于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论踝关节骨折伴胫腓损伤患者予以缝线环抱Nice结弹性固定,有助于提高手术优良率,改善踝关节功能、围术期情况,并发症发生率低。
Objective To explore the application value of suture embracing Nice knot elastic fixation in patients with ankle fractures accompanied by tibiofibular injuries.Methods One hundred and twenty patients with ankle fractures and tibiofibular injuries in Mentougou District Hospital from January to December 2024 were selected and randomly divided into an observation group and a control group,with 60 patients in each group,by a random number table method.The control group accepted cortical bone screw fixation,while the observation group accepted suture wrapped Nice knot elastic fixation.The excellent rate,joint morphology and structure,and ankle joint function indicators were compared between the two groups.Results There were no statistically significant differences in operation duration,fracture healing time,intraoperative blood loss,time to ambulation,and hospital stay between the two groups(P>0.05).The time to full weight-bearing in the observation group was shorter than that in the control group(P<0.05).The excellent and good rate in the observation group was higher than that in the control group(P<0.05).Three months postoperatively,the levels of inferior tibiofibular joint space,medial joint space,and ankle mortise width difference in both groups decreased compared to pre-operative measurements,while the tibiofibular overlap increased compared to pre-operative measurements.However,there were no statistically significant differences between the groups(P>0.05).Three months postoperatively,the ankle joint function scores,dorsiflexion and plantarflexion degrees of the ankle joint in the observation group were higher than those in the control group(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion For patients with ankle fractures accompanied by tibiofibular injuries,using suture wrapped Nice knot elastic fixation can help improve the surgical success rate,ankle joint function,perioperative conditions,and reduce the incidence of complications.
