摘要
目的:探讨四川省某大型公立医院的某分院区在医院感染“同质化”监测前后,医院感染发病率及感染部位构成的变化,为分院区医院感染管理工作持续优化提供依据。方法:以2020~2022年为“同质化”监测前组,2023~2024年为“同质化”监测后组,调查获取该分院区“同质化”监测前、后医院感染发病率及感染部位构成比,通过描述性分析和χ^(2)检验,比较分院区“同质化”监测前、后指标变化,及“同质化”监测后与主院区对应指标的差异。结果:分院区共监测到医院感染病例517例次,医院感染发病率为0.32%。分院区“同质化”监测后组发病率明显高于“同质化”监测前组(P<0.001)及主院区的“同质化”监测后组(P<0.005)。“同质化”监测前、后组,医院感染发病率排名前3的科室及位次不同,且部分科室的发病率有差异(P<0.001);感染部位构成比有差异(P<0.001),且与主院区的“同质化”监测后组相比也有差异(P<0.001)。结论:医院感染监测系统在分院区实现了“同质化”运行,为分院区医院感染精准防控提供科学依据。建议开展多中心研究,探索并建立适合国情的“一院多区”医院感染同质化监测体系,指导医院感染防控资源的优化配置,实现多院区医院的医院感染管理的精细化、科学化发展。
Objective To analyze the change of incidence and infection sites constituent ratio of healthcare associated infection before and after homogenous surveillance in a branch district of a general public hospital in Sichuan,and establish a foundation for improving management of healthcare-associated infection in branch district.Methods Data of incidence and infection sites constituent ratio of healthcare associated infection in two groups of before and after homogenous surveillance were investigated,and were comparatively analyzed by using descriptive analysis and chi-square test.Results A total of 517 cases of healthcare associated infection were detected at this district,with a incidence rate of 0.32%.The incidence rate in post-surveillance group was significantly higher than that in pre-surveillance group at the district(P<0.001),and also higher than that in post-surveillance group at main district(P<0.005).The top 3 incidence rates of departments in pre-surveillance and post-surveillance group were different,and the difference of incidence rates in some top 3 departments was significant(P<0.001).The infection sites constituent ratio in pre-surveillance and post-surveillance group were significantly different(P<0.001),and the difference of post-surveillance group between the district and main district was also significantly significant(P<0.001).Conclusion The homogenous surveillance of healthcare associated infection at the district can provide scientific evidence for precision prevention and control of healthcare-asso-ciated infection.It is suggested to conduct multi-center research aimed at exploring and establishing a standardized homogenous monitoring system for healthcare-associated infection within the"multiple districts in one hospital"model that is adaptable to China's national conditions,which should be helpful to guide the optimal allocation of healthcare-associated infection resources,and promote the refined and scientific management of healthcare-associated infection in hospital with multiple districts.
