摘要
目的探讨化瘀接骨汤用于骨质疏松性胸腰椎骨折术后的效果以及对患者骨密度和骨代谢水平的影响。方法回顾性分析开封市第二中医院2021年1月—2023年12月收治的骨质疏松性胸腰椎骨折患者100例,均应用椎体成形术治疗,术后以碳酸钙D3片治疗的患者为对照组(n=45),以中西医结合的方式化瘀接骨汤联合碳酸钙D3治疗的患者为观察组(n=55),对两组治疗8个周期后的疗效、骨密度及骨代谢指标进行回顾性分析比较。结果观察组治疗总有效率94.55%高于对照组73.33%(P<0.05);观察组腰酸背痛、舌质暗红、痿软乏力和神疲目眩等中医症候积分是(3.10±0.52)分、(1.15±0.26)分、(2.86±0.65)分、(1.32±0.35)分,均低于对照组(t=7.694、12.163、8.677、7.701;均P<0.001);观察组股骨颈骨密度、粗隆间骨密度和腰椎骨密度等骨密度水平是(0.82±0.10)g/cm^(2)、(0.57±0.13)g/cm^(2)、(0.90±0.12)g/cm^(2),均高于对照组(t=3.743、2.596、2.963;P<0.001、0.011、0.004);观察组OPG、BGP和ALP水平是(398.96±42.36)pg/mL、(9.00±1.25)μg/L、(265.90±26.35)U/L,均高于对照组(t=8.964、2.485、12.132;P<0.001、0.015、<0.001)。结论化瘀接骨汤可提升骨质疏松性胸腰椎骨折患者骨密度,改善骨代谢,对于加快患者术后康复进度有着重要的作用。
Objective To investigate the effect of Huayu Jigu decoction on osteoporotic thoracolumbar fracture and the effect on bone mineral density and bone metabolism.Methods A total of 100 patients with osteoporotic thoracolumbar fractures admitted to Kaifeng Second Hospital of Traditional Chinese Medicine from January 2021 to December 2023 were retrospectively analyzed,all of whom were treated with vertebroplasty.The patients treated with calcium carbonate D3 tablets after surgery were selected as the control group(n=45),and the patients treated with Huashu Jiegu Decoction combined with calcium carbonate D3 by combining traditional Chinese and Western medicine were selected as the observation group(n=55).The curative effect,bone mineral density and bone metabolism indexes of the two groups after 8 cycles of treatment were retrospectively analyzed and compared.Results The total effective rate of the observation group was 94.55%,which was higher than 73.33%of the control group(P<0.05).The TCM syndrome scores of backache,dark red tongue,weakness,weakness and fatigue in the observation group were(3.10±0.52)points,(1.15±0.26)points,(2.86±0.65)points and(1.32±0.35)points,which were lower than those in the control group(t=7.694,12.163,8.677,7.701;均P<0.001).The levels of femoral neck bone mineral density,intertrochanteric bone mineral density and lumbar spine bone mineral density in the observation group were(0.82±0.10)g/cm^(2),(0.57±0.13)g/cm^(2)and(0.90±0.12)g/cm^(2),which were higher than those in the control group(t=3.743,2.596,2.963;P<0.001,0.011,0.004).The levels of OPG,BGP and ALP in the observation group were(398.96±42.36)pg/mL,(9.00±1.25)μg/L and(265.90±26.35)U/L,which were higher than those in the control group(t=8.964,2.485,12.132;P<0.001,0.015,<0.001).Conclusion Huayu Jigu decoction can increase bone mineral density and improve bone metabolism in patients with osteoporotic thoracolumbar fracture,which plays an important role in speeding up postoperative rehabilitation.
