摘要
目的:探讨冰山理论指导下的综合护理对人工肝血浆置换(ALPE)治疗肝衰竭患者短期预后的影响.方法:选取2023年1月至2024年7月于本院行ALPE治疗的102例肝衰竭患者为研究对象.按护理方法,将其分为研究组(n=51,实施冰山理论指导下的综合护理)和对照组(n=51,实施常规护理).比较两组患者肝功能、遵医行为及短期预后.结果:护理后,研究组患者总胆红素(TBil)、丙氨酸氨基转移酶(ALT)及天冬氨酸氨基转移酶(AST)水平均低于对照组,差异均有统计学意义(P<0.05).研究组患者遵医率(47/51,92.16%)高于对照组(39/51,76.47%),差异有统计学意义(P<0.05).随访1个月、3个月及6个月时,研究组患者终末期肝病模型评分(MELD)评分均低于对照组,差异均有统计学意义(P<0.05).结论:冰山理论指导下的综合护理可改善行ALPE治疗的肝衰竭患者的肝功能,遵医行为及短期预后.
Objective:To explore the effect of comprehensive nursing guided by the iceberg theory on the short-term prognosis of patients with liver failure undergoing artificial liver plasma exchange(ALPE).Methods:A total of 102 patients with liver failure undergoing ALPE treatment in our hospital from January 2023 to July 2024 were selected as study subjects.According to the nursing method,they were divided into study group(n=51,received comprehensive nursing under the guidance of iceberg theory)and control group(n=51,received routine nursing).The liver function,compliance behavior and short-term prognosis were compared between the two groups.Results:After the nursing care,the levels of total bilirubin(TBil),alanine aminotransferase(ALT),aspartate aminotransferase(AST)in study group were lower than those in control group,and the differences were statistically significant(P<O.05).The compliance rate of study group(47/51,92.16%)was higher than that of control group(39/51,76.47%),and the difference was statistically significant(P<0.05).At 1 month,3 months and 6 months of follow-up visits,the scores of model for endstage liver disease(MELD)of study group were lower than those of control group,and the differences were statistically significant(P<O.05).Conclusion:Comprehensive nursing guided by the iceberg theory can improve the liver function,compliance with medical advice and short-term prognosis in patients with liver failure undergoing ALPE treatment.
