摘要
目的:探讨血管紧张素转换酶抑制剂(ACEI)治疗免疫球蛋白A(IgA)肾病的疗效及对血清肌酐水平的影响.方法:选取2021年7月至2024年6月本院收治的112例IgA肾病患者为研究对象.依据治疗方法,将其分为观察组(n=56,采用ACEI类药物治疗)和对照组(n=56,采用双嘧达莫片治疗).比较两组患者的临床疗效、肾功能指标、24h尿蛋白定量及不良反应发生率.结果:观察组患者临床总有效率高于对照组,差异有统计学意义(P<0.05).治疗3个月后,两组患者血清肌酐、尿素氮及24h尿蛋白定量均较治疗前降低,且观察组上述指标均低于对照组,差异均有统计学意义(P<0.05).观察组患者不良反应发生率(5.36%)低于对照组(17.86%),差异有统计学意义(P<0.05).结论:ACEI治疗IgA肾病疗效显著,可有效改善肾功能及减少24h尿蛋白排泄,且不良反应发生率更低,安全性良好.
Objective:To explore the efficacy of angiotensin-converting enzyme inhibitors(ACEI)in the treatment of immunoglobulin A(IgA)nephropathy and their effect on serum creatinine levels.Methods:A total of 112 patients with IgA nephropathy admitted to our hospital between July 2021 and June 2024 were enrolled as study subjects.According to the therapeutic regimen,they were divided into observation group(n=56,treated with ACEI)and control group(n=56,treated with dipyridamole tablets).The clinical efficacy,renal function parameters,24-hour urinary protein excretion,and incidence of adverse reactions were compared between the two groups.Results:The overall clinical effective rate in observation group was higher than that in control group,and the difference was statistically significant(P<0.05).After 3 months of treatment,serum creatinine,blood urea nitrogen,and 24-hour urinary protein excretion decreased compared to pre-treatment levels in both groups,and these parameters in observation group were lower than those in control group,and the differences were statistically significant(P<0.05).The incidence of adverse reactions in observation group(5.36%)was lower than that in control group(17.86%),and the difference was statistically significant(P<0.05).Conclusion:ACEI demonstrates significant efficacy in the treatment of IgA nephropathy.It can effectively ameliorate renal function and reduce 24-hour urinary protein excretion,with a lower incidence of adverse reactions and favorable safety profile.
