摘要
目的:探讨经皮椎间孔镜辅助治疗对腰椎椎体后缘离断症患者疼痛与椎管矢状径的影响。方法:选取2022年5月至2024年4月本院收治的118例腰4~5椎体后缘离断症患者为研究对象。根据治疗方式,将其分为观察组(n=59,行经皮椎间孔镜治疗)和对照组(n=59,行传统开窗术治疗)。比较两组患者手术相关指标、疼痛程度[采用视觉模拟量表(VAS)评估]功能状态[采用Oswestry功能障碍指数(ODI)评估]以及影像学参数。结果:观察组患者手术切口长度小于对照组,手术时间及住院时间均短于对照组,差异有统计学意义(P<0.05)术后1d及术后1个月,观察组患者VASODI评分均低于对照组,且术后3个月,观察组患者ODI评分仍低于对照组,差异均有统计学意义(P<0.05)。术后,观察组患者椎管正中矢状径大于对照组,差异有统计学意义(P<0.05)。结论:相较于传统开放手术,经皮椎间孔镜辅助治疗可更有效降低腰4~5椎体后缘离断症患者的VAS评分,改善椎管矢状径。
Objective:To explore the effect of percutaneous transforaminal endoscopy assisted treatment on pain and the sagittal diameter of the spinal canal in patients with posterior ring apophysis fracture.Methods:A total of 118 patients with L4~L5 posterior marginal detachment admitted to our hospital from May 2022 to April 2024 were enrolled as study subjects.According to the treatment modalities,they were divided into observation group(n=59,treated with percutaneous transforaminal endoscopy assisted treatment)and control group(n=59,treated with traditional open fenestration surgery).The surgical indicators,pain level[assessed by visual analogue scale(VAS)],functional status[assessed by Oswestry disability index(ODI)],and imaging parameters between the two groups were compared.Results:The incision length in observation group was shorter than that in control group,and the operative time and hospital stay were shorter in observation group compared to control group,and the differences were statistically significant(P<0.05).At postoperative day 1 and 1 month,both VAS and ODI scores in observation group were lower than those in control group,and at postoperative 3 months,the ODI score in observation group remained lower than that in control group,and the differences were statistically significant(P<0.05).Postoperatively,the median sagittal diameter of the spinal canal in observation group was larger than that in control group,and the difference was statistically significant(P<0.05).Conclusion:Compared with traditional open surgery,percutaneous transforaminal endoscopy assisted treatment can more effectively reduce the VAS score and improve the sagittal diameter of the spinal canal in patients with L4~L5 posterior marginal detachment.
