上肢等速肌力训练联合腹式呼吸训练对脑卒中患者肺功能及运动耐力的影响

Effect of isokinetic muscle strength training of upper limbs combined with abdominal breathing training on pulmonary function and motor endurance in stroke patients

摘要 目的:回顾性分析上肢等速肌力训练联合腹式呼吸训练在脑卒中患者中的应用效果。方法:收集我院2023年7月-2024年8月108例脑卒中患者临床资料,1:1匹配设计根据康复训练方法不同分为对照组(n=54)、研究组(n=54),对照组采用腹式呼吸训练,研究组联合上肢等速肌力训练。对比两组患侧肩内收肌、肘屈肌和腕屈肌的肌张力、运动耐力、上肢运动功能、肺功能。结果:干预后,研究组患侧肩内收肌、肘屈肌和腕屈肌的Ashworth量表(Modified Ashworth Scale,MAS)评分均高于对照组(P<0.05);干预后,研究组6 min移圈试验(6-minute walk test,6 PBRT)圈数高于对照组(P<0.05);干预后,研究组上肢Fugl-Meyer运动功能评定量表(Fugl-Meyer Assessment Upper Extremity Scale,FMA-UE)评分高于对照组(P<0.05);干预后,两组最大呼气压(Maximum Expiratory Pressure,MEP)、最大吸气压(Maximal inspiratory pressure,MIP)、用力肺活量(Forced vital capacity,FVC)水平均明显升高,且研究组高于对照组(P<0.05)。结论:上肢等速肌力训练联合腹式呼吸训练可促进脑卒中患者上肢运动功能恢复,改善其肺功能,提高运动耐力。 Objective:To retrospectively analyze the application effect of isokinetic muscle strength training combined with abdominal breathing training on upper limb in stroke patients.Methods:Clinical data of 108 stroke patients from our hospital from July 2023 to August 2024 were collected.According to the 1:1 matching design,the patients were divided into a control group(n=54)and a study group(n=54)based on different rehabilitation training methods.The control group underwent abdominal breathing training,while the study group underwent isokinetic muscle strength training of the upper limbs.The muscle tension,motor endurance,upper limb motor function,mechanical indicators of the affected limb,and pulmonary function of the affected side of the shoulder adductor,elbow flexor,and wrist flexor were compared between the two groups.Results:After intervention,the Ashworth Scale(MAS)scores of the affected side of the study group for the adductors of the shoulder,elbow flexors,and wrist flexors were higher than those of the control group(P<0.05).After intervention,the number of laps in the 6-minute lap test(6 PBRT)in the study group was higher than that in the control group(P<0.05).After intervention,the Fugl-Meyer Assessment of Upper Extremity(FMA-UE)score in the study group was higher than that in the control group(P<0.05).After intervention,the levels of maximum expiratory pressure(MEP),maximum inspiratory pressure(MIP),and forced vital capacity(FVC)in both groups increased significantly,and the study group was higher than the control group(P<0.05).Conclusion:Isokinetic muscle strength training of the upper limbs combined with abdominal breathing training can promote the recovery of upper limb motor function in stroke patients,improve their pulmonary function,and enhance their exercise endurance.
