摘要
目的系统检索、评价并总结成人肛瘘患者术后创面管理的最佳证据,为临床医务人员提供循证依据。方法基于“6S”证据模型,系统检索国内外数据库及相关专业网站关于肛瘘术后创面管理的相关文献,检索时限为建库至2025年2月15日。对纳入的文献进行评价并汇总证据。结果共纳入14篇文献,包括2篇指南、7篇专家共识、1篇临床决策、3篇系统评价和1篇随机对照试验。从多学科团队协作、疼痛管理、物理干预治疗、中医特色疗法、创面换药护理、营养支持管理、运动管理、健康教育与随访8个方面总结出了26条最佳证据。结论基于“6S”证据模型总结的成人肛瘘患者术后创面管理最佳证据,可为临床医务人员提供循证依据,从而降低患者肛瘘复发率。
Objective This study aimed to collect,appraise,and synthesize the optimal evidence regarding postoperative wound management for adult patients with anal fistula,thereby offering an evidence-based reference for healthcare providers.Methods Adopting the"6S"evidence-based model,a comprehensive search was conducted across domestic and international databases,as well as relevant professional websites,for evidence related to wound management following anal fistula surgery.The search span covered the period from the database inception to February 15,2025.Subsequently,the included literatures were critically appraised,and the evidence was systematically summarized.Results A total of 14 articles were included,encompassing 2 guidelines,7 expert consensus statements,1 clinical decision-making document,3 systematic reviews,and 1 randomized controlled trial(RCT).A total of 26 pieces of best evidence were summarized from 8 aspects,including multi-disciplinary team cooperation,comprehensive pain management,physical therapy intervention,traditional Chinese medicine specialized therapies,wound dressing change nursing,nutritional support management,exercise management,health education and follow-up.Conclusion The optimal evidence for postoperative wound management in adult anal fistula patients,summarized using the"6S"evidence-based model,can serve as a reliable evidence-based reference for clinical practitioners.This evidence is expected to facilitate wound healing and decrease the recurrence rate of anal fistula.
