摘要
目的:探讨保留括约肌挂线术治疗肛周脓肿的临床效果。方法:选取2022年1月—2024年1月扬州市妇幼保健院收治的60例肛周脓肿患者进行研究,随机分为两组,每组30例。对照组行一期根治术,观察组行保留括约肌挂线术。比较两组肛肠动力学指标、临床指标、疼痛程度、肛门功能、术后并发症发生风险。结果:治疗后,两组直肠静息压、肛管静息压、肛管最大收缩压较术前低(P<0.05),且观察组低于对照组(P<0.001)。两组手术时间对比,无统计学差异(P>0.05)。观察组创面愈合时间比对照组早,住院时间较对照组短(P<0.001)。术后1 d、术后7 d,两组视觉模拟评分法、肛门失禁评分量表评分较术前低(P<0.05),且观察组较对照组低(P<0.001)。观察组术后并发症总发生率较对照组低(P=0.044)。结论:保留括约肌挂线术治疗肛周脓肿的临床效果较好,可减轻患者术后疼痛,保护肛门功能,减少术后并发症,促进患者恢复。
Objective:To explore the clinical effect of sphincter-sparing cutting with thread ligation in the treatment of perianal abscess.Methods:A total of 60 patients with perianal abscess admitted to Yangzhou Women and Children’s Healthcare Hospital from January 2022 to January 2024 were selected for study.They were randomly divided into two groups,with 30 cases in each group.The control group underwent one-stage radical surgery,and the observation group underwent sphincter-sparing cutting with thread ligation.The anorectal dynamic indexes,clinical indexes,pain degree,anal function,and risk of postoperative compli⁃cations were compared between the two groups.Results:After treatment,the rectal resting pressure,anal canal resting pressure and anal canal maximum systolic pressure in the two groups were lower than those before surgery(P<0.05),and those parameters in the observation group were lower than those in the control group(P<0.001).There was no statistically significant difference in operation time between the two groups(P>0.05).The wound healing time in the observation group was earlier than that in the con⁃trol group,and the length of hospital stay in the observation group was shorter than that in the control group(P<0.001).On 1 d and 7 d after surgery,the scores of visual analogue scale and anal incontinence rating scale in the two groups were lower than those be⁃fore surgery(P<0.05),and the scores in the observation group were lower than those in the control group(P<0.001).The total inci⁃dence of postoperative complications in the observation group was lower than that in the control group(P=0.044).Conclusion:The sphincter-sparing cutting with thread ligation shows better clinical effect in the treatment of perianal abscess,and can relieve postoperative pain of patients,protect anal function,reduce postoperative complications,and promote the recovery of patients.
