鞘内注射甲氨蝶呤诱发脊髓侧索病变一例报告

摘要 患者男,57岁,因“进行性双下肢麻木无力伴大小便障碍20 d”于2023年12月5日收入首都医科大学宣武医院神经内科。2023年6月,患者外院确诊为急性淋巴细胞白血病,并接受规范化治疗。2023年7月开始接受鞘内注射治疗,方案为甲氨蝶呤(methotrexate, MTX) 15 mg+阿糖胞苷50 mg+地塞米松10 mg,累计共10次治疗。末次MTX鞘内注射2周后,患者在无明确外伤感染诱因下出现双下肢无力,症状进行性加重,至独自行走困难,伴双足底麻木,无肢体疼痛。
