摘要
目的分析可弯曲负压输尿管鞘联合输尿管软镜钬激光碎石术在肾输尿管结石中的作用。方法纳入该院泌尿外科2024年5月至2025年5月的50例肾输尿管结石患者,根据手术方法分组。予以对照组(n=25)普通输尿管鞘联合输尿管软镜钬激光碎石术,观察组(n=25)接受可弯曲负压输尿管鞘联合输尿管软镜钬激光碎石术,比较两组结石清除率、临床疗效、肾功能和生活质量。结果观察组治疗结石清除率和总有效率更高,组间差异明显(P<0.05);两组治疗前肾功能和生活质量组间对比差异无统计学意义(P>0.05),经治疗,两组血肌酐、尿素氮、β2微球蛋白上升,肾小球滤过率下降,且观察组各项指标变化幅度更小;生活质量评分上升,观察组评分更高,组内、组间比较差异均有统计学意义(P<0.05)。结论予以肾输尿管结石可弯曲负压输尿管鞘联合输尿管软镜钬激光碎石术,可提高结石清除效果,改善肾功能,提高生活质量。
Objective To analyze the role of the flexible negative pressure ureteral sheath combined with ureteroscopic holmium laser lithotripsy in renal and ureteral calculi.Methods Fifty patients with renal and ureteral calculi from the urological department of our hospital from May 2024 to May 2025 were included.They were grouped according to the surgical method.The control group(n=25)received conventional ureteral sheath combined with ureteroscopic holmium laser lithotripsy,while the observation group(n=25)underwent flexible negative pressure ureteral sheath combined with ureteroscopic holmium laser lithotripsy.The stone clearance rate,clinical efficacy,renal function,and quality of life of the two groups were compared.Results The stone clearance rate and total effective rate of the observation group were higher,and the difference between the groups was significant(P<0.05).There was no statistically significant difference in renal function and quality of life between the two groups before treatment(P>0.05).After treatment,the levels of serum creatinine,urea nitrogen,andβ2 microglobulin increased,and the glomerular filtration rate decreased in both groups,and the changes in the observation group were smaller.The quality of life score increased,and the score of the observation group was higher.There were statistically significant differences in both within-group and between-group comparisons(P<0.05).Conclusion Using the flexible negative pressure ureteral sheath combined with ureteroscopic holmium laser lithotripsy for renal and ureteral calculi can improve the stone clearance effect,and also has an improvement effect on quality of life and renal function.
