两种检测设备在动脉血电解质检测中的正确度性能对比评价

摘要 目的:对比评价血气分析仪直接离子选择电极法与传统全自动生化分析仪间接离子选择电极法对临床患者动脉血钾(K^(+))、钠(Na^(+))、氯(Cl^(-))检测结果,以评估血气分析仪电解质检测结果满足临床需求的性能。方法:选取2023年5月至6月于中国人民解放军海军第九七一医院收集获得的100例临床患者动脉血样本,分别采用Radiometer ABL90 FLEX型血气分析仪和Roche Cobas 8000 c702型全自动生化分析仪,对临床患者动脉血K^(+)、Na^(+)、Cl^(-)检测结果进行正确度评价,并进行差异比较及离群值检验。参照美国临床和实验室标准化协会(CLSI)EP9-A3指南要求,分析两种仪器检测结果的相关性和偏倚,评估结果可比性。结果:血气分析仪与全自动生化分析仪对K^(+)、Na^(+)、Cl^(-)的检测结果差异无统计学意义(P>0.05)。两种仪器对K^(+)、Na^(+)、Cl^(-)检测的Pearson相关性分析结果均呈正相关性(r=0.911、0.920、0.892,P<0.05),两种方法检测K^(+)、Na^(+)、Cl^(-)结果一致性良好,预期偏差可接受。结论:血气分析仪的电解质检测结果能够满足临床需求,具有较高的准确性和可靠性,且血气分析仪具备操作简便、检测速度快、体积小等特点,其广泛应用有助于临床快速诊疗和病情监测工作。 Objective:To compare and analyze the detected results of arterial blood potassium(K^(+)),sodium(Na^(+)),and chloride(Cl^(-))of using direct ion-selective electrode(ISE)of blood gas analyzer and indirect ISE of full automatic biochemistry analyzer,so as to assess the performance of blood gas analyzer in meeting clinical requirement.Methods:A total of 100 arterial blood samples of clinical patients were collected at the 971th Hospital the Navy of the Chinese PLA from May to June 2023.The Radiometer ABL90 FLEX blood gas analyzer and the Roche Cobas 8000 c702 full automatic biochemistry analyzer were adopted respectively to conduct trueness evaluation for arterial blood potassium(K^(+)),sodium(Na^(+)),and chloride(Cl^(-))of clinical patients.The differences of them were compared,and the outlier was tested.In accordance with the CLSI EP09-A3 guideline,the correlation and bias of the tested results between the two methods were analyzed,and the comparability of the results was assessed.Results:There were not significant differences at the results of K^(+),Na^(+),and Cl^(-)between the blood gas analyzer and the full automatic biochemistry analyzer(P>0.05).Pearson correlation analysis demonstrated positive correlations for the results of K^(+),Na^(+),and Cl^(-)between the two instruments(r=0.911,0.920,0.892,P<0.05).There were favorable consistencies at the tests for K^(+),Na^(+),and Cl^(-)between two kinds of methods,and the expected deviation was acceptable.Conclusion:The electrolyte results of the blood gas analyzer can meet clinical requirement,which has higher accuracy and reliability.Blood gas analyzer has a series of advantages,such as operational simplicity,rapid detection velocity,and small volume,which wide application can help to quickly diagnose and treat in clinical practice,and monitor disease condition.
