基于工作链和可拓物元模型的医疗设备安全运行风险评估模型在医疗设备管理中的应用价值

摘要 目的:基于工作链和可拓物元模型构建医疗设备安全运行风险评估模型,探讨其在医院内分泌科设备安全运行管理中的应用效果。方法:基于工作链建立设备运行安全风险评估指标体系,采用可拓物元模型对内分泌科医疗设备运行数据进行综合评定,开展安全自查和针对性干预,构建医疗设备安全运行风险评估模型(简称风险评估模型)。选取2023年至2024年合肥市第二人民医院内分泌科临床在用的56台医疗设备,2023年用常规巡检和经验管理模式(常规管理模式)管理设备,2024年用风险评估模型管理模式(风险评估管理模式)管理设备,比较两种管理模式的设备风险事件发生率和管理质量的差异。采用自制问卷对参与设备操作的26名医护人员进行安全知识和操作技能考核,比较医护人员对两种管理模式的安全认知能力考核得分。结果:采用风险评估管理模式的56台设备操作设置故障、数据丢失或异常和校准问题风险事件台数分别为1台(占1.79%)、2台(占3.57%)和1台(占1.79%),均低于常规管理模式的8(占14.29%)、9(占16.07%)和8(占14.29%),差异均有统计学意义(χ^(2)=4.350、4.939、4.350,P<0.05)。采用风险评估管理模式的医护人员在设备理论知识、安全使用、管理意识和故障判定考核中得分均高于常规管理模式的医护人员,差异均有统计学意义(t=8.418、12.829、31.441、9.907,P<0.05),且风险评估管理模式的设备性能控制质量和设备运行质量评分均高于常规管理模式,差异均有统计学意义(t=24.437、28.941,P<0.05)。结论:基于工作链和可拓物元模型的医疗设备安全运行风险评估模型应用于医院内分泌科医疗设备管理,能够降低设备风险事件发生率,提升设备操作人员的设备安全认知能力,提高设备管理质量。 Objective:To construct a risk assessment model based on work chain and extensible matter element model for safe operation of medical equipment,and explore its application effect in the management for safe operation of equipment at department of endocrinology of hospitals.Methods:To establish a risk assessment indicator system of operational safety on the basis of work chain.The extensible matter element model was adopted to conduct comprehensive evaluation for the operational data of medical equipment of the department of endocrinology,and carry out safety self-examination and targeted intervention,so as to construct the risk assessment model of safe operation of medical equipment(risk assessment model).A total of 56 medical equipment that were used at the department of endocrinology of Hefei Second People's Hospital from 2023 to 2024 were selected,and the routine inspection and experience management mode(routine management mode)was adopted to manage equipment in 2023,and the management mode based on risk assessment model(risk assessment management mode)was adopted to manage equipment in 2024.The differences of incidence of risk events,and management quality of equipment between the two management modes were compared.A self-made questionnaire was adopted to assess safety knowledge and operational skills of 26 medical staffs who participated in operating equipment.The assessment scores of the ability of safety cognition of medical staffs for two kinds of management modes were compared.Results:The numbers of risk events included the fault due to setting up operation,data loss or abnormalities,and calibration issues in 56 equipment that were managed by risk assessment management mode were respectively 1 equipment(1.79%),2 equipment(3.57%),and 1 equipment(1.79%),all of which were significantly lower than[(14.29%),9 equipment(16.07%),and 8 equipment(14.29%)]of routine management mode,and the differences were statistically significant(χ^(2)=4.350,4.939,4.350,P<0.05).The assessment scores of medical staffs on the theoretical knowledge,safe use,management awareness,and fault judgment of equipment who adopted risk assessment management mode were significantly higher than those who adopted routine management mode,and the differences were statistically significant(t=8.418,12.829,31.441,9.907,P<0.05).In addition,the scores of performance control quality and operation quality of equipment of risk assessment management mode were significantly higher than those of routine management mode,and the differences were statistically significant(t=24.437,28.941,P<0.05).Conclusion:The application of the risk assessment model based on the work chain and extensible matter element model for safe operation of medical equipment in the management for medical equipment of endocrinology department of hospital can reduce the incidence of risk events of equipment,and improve the cognition ability of operators for equipment,and enhance the quality of managing equipment.
