减数正畸+牙周系统治疗在重度牙周炎错畸(牙合)形患者治疗中的应用研究

摘要 目的探究重度牙周炎错(牙合)畸形患者经减数正畸、牙周系统联合治疗后的临床效果以及应用价值。方法将2022年1月至2024年12月郑州市口腔医院收治92例重度牙周炎错(牙合)畸形患者(共192颗减数牙)作为研究对象,均给予减数正畸联合牙周系统治疗,观察组选取384颗减数牙邻牙,选取同颌对侧同名牙384颗减数牙邻牙作为对照组,比较两组患者牙齿牙周相关指标,牙槽骨高度(RBH),牙周致病菌水平和龈沟液炎症因子变化情况。结果矫治后,观察组患者的减数牙邻牙牙周相关指标[牙龈指数(1.17±0.32)、探诊深度(3.02±0.39)mm]均低于对照组的(1.50±0.47)、(3.15±0.53)mm、相比,差异有统计学意义(t=11.373,P<0.001;t=3.871,P<0.001);矫治后,观察组的牙槽骨高度高于对照组同期,差异有统计学意义(P<0.05)。矫治后,观察组的牙周致病菌数量[塞斯坦纳菌(2.26±0.25)μL/mL、牙龈卟啉单胞菌(3.87±0.42)μL/mL、具核梭杆菌(2.65±0.38)μL/mL]均低于对照组(3.08±0.34)μL/mL、(4.32±0.63)μL/mL、(3.08±0.47)μL/mL,差异有统计学意义(t=18.637,P<0.001;t=5.701,P<0.001;t=13.941,P<0.001);矫治后,观察组的牙周炎症因子TNF-α、IL-6、IL-1β降低,且低于对照组,差异有统计学意义(P<0.05)。结论减数正畸与牙周系统治疗相结合,未对患者牙周组织指标产生显著影响,牙槽骨高度有所提高,牙周细菌感染情况、牙周炎症反应均减轻,可临床推广应用。 Objective To explore the clinical efficacy and application value of combined treatment of meiotic orthodontics and periodontal system in patients with severe periodontitis malocclusion.Method:A total of 92 patients with severe periodontitis and malocclusion(192 teeth)admitted to Zhengzhou Stomatological Hospital from January 2022 to December 2024 were selected as the study subjects.They were all treated with orthodontic treatment combined with periodontal system.The observation group selected 384 adjacent teeth with reduced teeth,and the control group selected 384 adjacent teeth with the same name on the opposite side of the same jaw.The periodontal related indicators,alveolar bone height(RBH),periodontal pathogenic bacteria levels,and gingival crevicular fluid inflammatory factors were compared between the two groups of patients.After orthodontic treatment,the periodontal related indicators of adjacent teeth in the observation group[probing depth(3.02±0.39)mm,gingival index(1.17±0.32)]were lower than those in the control group(3.15±0.53)mm,(1.50±0.47),and the differences were statistically significant(t=11.373,P<0.001;t=3.871,P<0.001);After correction,the height of alveolar bone in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After orthodontic treatment,the number of periodontal pathogenic bacteria in the observation group such as Serratia(2.26±0.25)μL/mL,Porphyromonas gingivalis(3.87±0.42)μL/mL,and Fusobacterium nucleatum(2.65±0.38)was significantly lower than that in the control group(3.08±0.34)μL/mL,(4.32±0.63)μL/mL,and(3.08±0.47)μL/mL,with statistical significance(t=18.637,P<0.001;t=5.701,P<0.001;t=13.941,P fl<0.001);After orthodontic treatment,the periodontal inflammation factors TNF-α,IL-6,and IL-1β in the observation group decreased and were lower than those in the control group,with statistical significance(P<0.05).Conclusions The combination of meiotic orthodontics and periodontal system treatment did not have a significant effect on the periodontal tissue indicators of patients.The height of alveolar bone was increased,and the incidence of periodontal bacterial infection and periodontitis were reduced.It can be clinically promoted and applied.
