卡式微柱凝胶免疫试验与低离子凝聚胺技术在新生儿ABO溶血病输血检验中的应用效果

摘要 目的:探讨卡式微柱凝胶免疫试验与低离子凝聚胺技术在新生儿ABO溶血病输血检验中的应用效果。方法:选取黔西南布依族苗族自治州人民医院2021年1月—2025年2月收治的72例新生儿ABO溶血病患儿,共交叉配血262次,低离子凝聚胺法及卡式微柱凝胶免疫试验法各131次。以新生儿最终输血效果为“金标准”,对比两种交叉配血技术的诊断效能、检验时间和检测结果。结果:72例患儿最终成功输血66例,成功率91.67%;失败6例,失败率8.33%;两种技术诊断灵敏度、特异度、准确度比较,差异无统计学意义(P>0.05);低离子凝聚胺法检验时间低于卡式微柱凝胶免疫试验法(P<0.001);卡式微柱凝胶免疫试验法主侧不合、主次侧不合阳性检出率高于低离子凝聚胺法(P<0.05);两种技术次侧不合阳性检出率比较,差异无统计学意义(P>0.05)。结论:低离子凝聚胺法检验时间短,适合紧急输血时快速筛查;卡式微柱凝胶法对血型不合抗体的检出能力更强,能提高输血安全性,适用于ABO溶血病等复杂情况。 Objective:To explore the application effect of card microcolumn gel immunoassay and low-ion coagulant amine technology in blood transfusion tests for neonatal ABO hemolytic disease.Methods:A total of 72 children with neonatal ABO hemolytic disease admitted to the People's Hospital of Qianxinan Buyi and Miao Autonomous Prefecture from January 2021 to February 2025 were selected.A total of 262 cross-matches were performed,with 131 matches each by the low-ion agglutamine method and the card microcolumn gel immunoassay method.Taking the final blood transfusion effect of newborns as the"gold standard",the diagnostic efficacy,test time and test results of the two cross-matching techniques were compared.Results:Among the 72 children patients,66 cases were successfully transfused with blood eventually,with a success rate of 91.67%.There were 6 failed cases,with a failure rate of 8.33%.There was no statistically significant difference in the diagnostic sensitivity,specificity and accuracy between the two techniques(P>0.05).The test time of the low-ion condensed amine method was shorter than that of the card microcolumn gel immunoassay method(P<0.001).The positive detection rates of primary and secondary side incompatibility by the card microcolumn gel immunoassay were higher than those by the low-ion condensed amine method(P<0.05).There was no statistically significant difference in the positive detection rate of secondary non-conformity between the two techniques(P>0.05).Conclusion:The low-ion coagulated amine method has a short testing time and is suitable for rapid screening during emergency blood transfusion.The card microcolumn gel method has a stronger detection ability for blood type incompatibility antibodies,which can improve the safety of blood transfusion and is suitable for complex conditions such as ABO hemolytic disease.
