摘要
目的:探讨基于清单管理的系统化鼻饲方案在预防气管切开鼻饲患者肺部感染中的应用效果。方法:选取2023年1月—2024年6月于深圳市大鹏新区南澳人民医院接受气管切开并行鼻饲治疗的112例患者为研究对象,随机分为常规组(56例)与清单组(56例)。常规组实施常规鼻饲护理干预,清单组采用基于清单管理的系统化鼻饲方案。比较两组肺部感染及恢复相关指标、营养代谢及气道清除功能指标。结果:清单组肺部感染发生率、感染时气道分泌物细菌培养阳性率低于常规组(P<0.05),肺部感染持续时间、机械通气时间、住院时间短于常规组(P<0.05)。干预后,清单组血清白蛋白、前白蛋白、转铁蛋白水平高于常规组(P<0.001),气道分泌物排出量低于常规组(P<0.001)。结论:基于清单管理的系统化鼻饲方案可有效降低气管切开鼻饲患者肺部感染发生率,缩短感染持续时间与住院周期,改善营养代谢指标及气道清除功能。
Objective:To explore the application effect of systematic nasal feeding scheme based on list management on preventing pulmonary infection in patients undergoing tracheotomy and nasal feeding.Methods:A total of 112 patients undergoing tracheotomy and nasal feeding at Nan'ao People's Hospital in Dapeng New District of Shenzhen City between January 2023 and June 2024 were selected as the study subjects.They were randomly divided into routine group(56 cases)and checklist group(56 cases).The routine group received routine nasal feeding nursing intervention,and the checklist group received systematic nasal feeding scheme based on list management.The indexes related to pulmonary infection and recovery,nutritional metabolism and airway clearance function were compared between the two groups.Results:Compared with the routine group,the checklist group had a lower incidence of pulmonary infection and positive rate of airway secretions by bacterial culture during infection(P<0.05),and a shorter duration of pulmonary infection,duration of mechanical ventilation,and length of hospital stay(P<0.05).After intervention,the levels of serum albumin,prealbumin,and transferrin in the checklist group were higher than those in the routine group(P<0.001),and the airway secretion discharge volume in the checklist group was lower than that in the routine group(P<0.001).Conclusion:The systematic nasal feeding scheme based on list management can effectively reduce the incidence of pulmonary infection in patients undergoing tracheotomy and nasal feeding,shorten the duration of infection and hospitalization period,and improve the nutritional metabolism indexes and airway clearance function.
