摘要
能谱CT通过双能量成像实现物质分离与定量分析,显著提升了结直肠癌诊疗的精准性。目前认为,能谱CT在术前分期、淋巴结转移鉴别、肝转移灶检测、肿瘤分化程度预测、脉管侵犯评估及疗效监测等方面具有重要价值。能谱CT的多参数成像能力为结直肠癌的形态和功能评估提供了有力工具,如碘浓度、有效原子序数和虚拟单能量图像,但其仍面临标准化不足、受肠道蠕动干扰、在大体型患者中应用受限等挑战。未来研究应聚焦于推动多模态影像融合、发展光子计数CT技术、开发靶向对比剂等方面,以实现更精准的分子成像和术中导航,从而推动结直肠癌个体化精准诊疗的发展。
Spectral CT achieves material separation and quantitative analysis through dual-energy imaging,significantly enhancing the accuracy of colorectal cancer diagnosis and treatment.At present,it is believed that spectral CT has significant value in preoperative staging,differentiation of lymph node metastasis,detection of liver metastases,prediction of tumor differentiation degree,assessment of vascular invasion and monitoring of therapeutic effect.The multi-parameter imaging capability of spectral CT provides a powerful tool for the morphological and functional assessment of colorectal cancer,such as iodine concentration,effective atomic number and virtual single-energy images,etc.,although it still faces challenges such as insufficient standardization,interference by intestinal peristalsis,and limited application in large-sized patients.Future research should focus on promoting multimodal image fusion,developing photon-counting CT technology,and develo ping targeted contrast agents,etc.,so as to achieve more precise molecular imaging and intraoperative navigation,thereby promoting the development of individualized and precise diagnosis and treatment of colorectal cancer.
