摘要
目的:探讨五行补泻针法结合俞募配穴治疗肝郁脾虚抑郁症的临床效果。方法:选取2022年7月—2023年1月福建中医药大学附属第二人民医院肝郁脾虚抑郁患者68例,按照随机数字表法进行划分为对照组和观察组,每组各34例。对照组为常规治疗+常规针刺疗法,观察组加用五行补泻针法结合俞募配穴针刺疗法,均连续治疗4周。比较两组各项临床指标。结果:治疗后组间比较,观察组总有效率更高,差异有统计学意义(P<0.05)。观察组汉密尔顿抑郁量表(HAMD)-17各项评分以及总评分、躯体症状严重程度量表(PHQ-15)评分均更低,一般自我效能感量表(GSES)评分更高,差异有统计学意义(P<0.05)。观察组肠道双歧杆菌、乳酸杆菌菌群数量更多,大肠埃希菌、肠球菌菌群数量更少,差异有统计学意义(P<0.05)。结论:五行补泻针法结合俞募配穴治疗肝郁脾虚抑郁症患者疗效确切,可以缓解患者抑郁情绪,提高患者躯体健康水平以及自我效能感,调节肠道菌群。
Objective:To explore the clinical effect of the Five-element tonifying and purging acupuncture method combined with the Yu Yupei acupoint in the treatment of depression caused by liver depression and spleen deficiency.Method:This study was a prospective study,including a total of 68 patients with liver-qi stagnation and spleen-deficiency depression,all selected from the Second Affiliated People's Hospital to Fujian University of Traditional Chinese Medicine.The selection was completed from July 2022 to January 2023.The patients were divided into two groups:the control group and the observation group—according to the random number table method,with 34 cases in each group.The control group received conventional treatment plus conventional acupuncture therapy,while the observation group additionally received the Five-element tonifying and reducing acupuncture method combined with theYu Yupei acupoint acupuncture therapy.Both groups underwent continuous treatment for 4 weeks.The clinical indicators of the two groups were compared.Result:After treatment,when comparing the groups,the total effective rate of the observation group was higher,with statistically significant difference(P<0.05).The scores of each item of the Hamilton Depression Scale(HAMD)-17,the total score,and the score of the Somatic Symptom Severity Scale(PHQ-15)in the observation group were all lower,while the score of the General Self-Efficacy Scale(GSES)was higher,with statistically significant difference(P<0.05).The number of Bifidobacterium and Lactobacillus flora in the observation group was higher,while the number of Escherichia coli and Enterococcus flora was lower,with statistically significant difference(P<0.05).Conclusion:The Five-element tonifying and purging acupuncture method combined with the Shu and Mu acupoints has a definite therapeutic effect on patients with liver depression and spleen deficiency depression.It can relieve patients'depressive mood,improve their physical health and self-efficacy,and regulate the intestinal flora.
