摘要
目的 观察自拟养血润肤方联合青鹏软膏封包治疗慢性难治性手部湿疹的临床疗效。方法 选取2021年9月—2023年2月在北京中医药大学第三附属医院皮肤科门诊就诊的慢性难治性手部湿疹患者68例,按随机数字表法分为对照组和治疗组,每组各34例。对照组予外涂青鹏软膏封包2 h,2次/d。治疗组予自拟养血润肤方浸浴20 min,1次/d,并外涂青鹏软膏封包2 h,2次/d。治疗周期均为4周,2周复诊一次。观察比较两组患者的临床疗效,治疗前后手部湿疹严重指数(Hand eczema severity index,HECSI)评分、瘙痒视觉模拟评分(Visual analogue scale,VAS)、皮肤病生活质量指数(Dermatology life quality index,DLQI)评分及安全性。结果 治疗后治疗组临床总有效率90.91%(30/33)明显高于对照组72.73%(24/33),差异有统计学意义(P<0.05)。治疗后两组患者HECSI评分、VAS评分和DLQI评分均较治疗前降低,差异有统计学意义(P<0.05);且治疗组HECSI评分、VAS评分和DLQI评分明显低于对照组,差异有统计学意义(P<0.05)。治疗期间,两组患者均未发生明显不良反应。结论 自拟养血润肤方联合青鹏软膏封包治疗慢性难治性手部湿疹能够有效改善皮损,缓解瘙痒症状,提高患者生活质量。
Objective To observe the clinical efficacy of the combined packet treatment of the self-formulated Yangxue Runfu Prescription with Qingpeng Ointment for chronic refractory hand eczema.Methods A total of 68 patients with chronic refractory hand eczema at the dermatology outpatient department of Beijing University of Chinese Medicine Third Affiliated Hospital from September 2021 to February 2023 were selected and divided into a treatment group and a control group by the random number table method,with 34 cases in each group.Both groups received a 2-hour packet treatment of Qingpeng Ointment twice a day,while the treatment group additionally received a 20-min bath of self-formulated Yangxue Runfu Prescription once a day.In a 4-week treatment cycle,the patients visited once every 2 weeks.After treatment,clinical efficacy was observed between the two groups,as well as parameters below before and after treatment:hand eczema severity index(HECSI)scores,visual analogue scale(VAS)scores,and dermatology life quality index(DLQI)scores.Results The total effective rate was 90.91%(30/33)in the treatment group,significantly higher than 72.73%(24/33)in the control group(P<0.05).HECSI scores,VAS scores,and DLQI scores were decreased in both groups(P<0.05),with these scores being significantly lower in the treatment group(P<0.05).During treatment,no significant adverse reaction occurred in either group.Conclusion For chronic refractory hand eczema,the combined packet treatment of self-formulated Yangxue Runfu Prescription with Qingpeng Ointment can effectively improve skin lesions and relieve itching symptoms,improving patients'quality of life.
