摘要
目的 探讨丹栀逍遥散联合针刺治疗肝郁脾虚型耳鸣的临床效果。方法 选取2023年11月—2024年11月泰安市中医医院收治的120例肝郁脾虚型耳鸣患者为研究对象。根据随机数字表法分为对照组与研究组,各60例。对照组接受针刺治疗,研究组则接受丹栀逍遥散联合针刺治疗。对比2组症状积分、耳鸣残疾量表(tinnitus disability inventory,THI)评分、耳鸣评价量表(tinnitus evaluation questionnaire,TEQ)分级及并发症发生率。结果 治疗后,研究组耳鸣、听力下降、眩晕、耳道瘙痒症状积分分别为(1.78±0.23)分、(1.23±0.13)分、(1.29±0.11)分、(1.14±0.19)分,均低于对照组的(2.69±0.19)分、(2.17±0.24)分、(2.13±0.26)分、(2.16±0.28)分,差异有统计学意义(P<0.001)。治疗后,研究组功能性评价、情感性评价、严重性评价评分为(11.27±1.13)分、(9.31±1.14)分、(3.46±0.27)分,均低于对照组的(16.56±2.24)分、(13.73±2.38)分、(6.33±1.15)分,差异有统计学意义(P<0.001)。治疗后,研究组TEQ分级优于对照组,差异有统计学意义(P<0.05)。治疗后,研究组并发症总发生率为5.00%(3/60),低于对照组的16.67%(10/60),差异有统计学意义(P<0.05)。结论 丹栀逍遥散联合针刺治疗肝郁脾虚型耳鸣的临床效果较好,能有效改善耳鸣症状,减轻病情严重程度,并降低并发症发生率。
Objective To explore the clinical effect of Danzhi Xiaoyao powder combined with acupuncture in the treatment of tinnitus of liver depression and spleen deficiency type.Methods A total of 120 patients with tinnitus of liver depression and spleen deficiency type admitted to Tai'an Traditional Chinese Medicine Hospital from November 2023 to November 2024 were selected as the research subjects.According to the random number table method,they were divided into the control group and the study group,with 60 cases in each group.The control group was treated with acupuncture treatment,while the study group was treated with Danzhi Xiaoyao powder combined with acupuncture treatment.The symptom scores,tinnitus disability inventory(THI)scores,tinnitus evaluation questionnaire(TEQ)grades and the incidence of complications were compared between the two groups.Results After treatment,the symptom scores of tinnitus,hearing loss,vertigo and pruritus of the ear canal in the study group were(1.78±0.23)points,(1.23±0.13)points,(1.29±0.11)points and(1.14±0.19)points respectively,which were lower than(2.69±0.19)points,(2.17±0.24)points,(2.13±0.26)points,(2.16±0.28)points in the control group,and the differences were statistically significant(P<0.001).After treatment,the scores of functional evaluation,affective evaluation and severity evaluation in the study group were(11.27±1.13)points,(9.31±1.14)points and(3.46±0.27)points,respectively,which were all lower than(16.56±2.24)points,(13.73±2.38)points and(6.33±1.15)points in the control group,the differences were statistically significant(P<0.001).After treatment,the TEQ grade of the study group was better than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the total incidence of complications in the study group was 5.00%(3/60),which was lower than 16.67%(10/60)in the control group,and the difference was statistically significant(P<0.05).Conclusion The clinical efficacy of Danzhi Xiaoyao powder combined with acupuncture in the treatment of tinnitus of liver depression and spleen deficiency type is remarkable,which can effectively improve the symptoms of tinnitus,reduce the severity of the disease,and reduce the incidence of complications.
