摘要
目的比较传统手工涂片和液基细胞学技术在甲状腺细针穿刺细胞学(FNAC)中的应用价值。方法随机选取2024年1月至2025年6月在贵港市中西医结合骨科医院接受FNAC的80例甲状腺结节标本,分别采用传统手工涂片与液基细胞学技术进行处理。比较两种方法在细胞形态、染色效果、染色优良率、阳性检出率及诊断一致率方面的差异。结果液基细胞学制片的细胞形态、染色效果均优于传统手工涂片。液基细胞学制片的染色优良率为88.75%,高于传统手工涂片的70.00%,差异具有统计学意义(P<0.05)。液基细胞学制片的标本阳性检出率为31.25%,高于传统手工涂片的23.75%,但差异无统计学意义(χ^(2)=1.13,P=0.29)。液基细胞学制片的诊断一致率为92.59%,高于传统手工涂片的70.37%,差异具有统计学意义(χ^(2)=4.42,P=0.04)。与传统手工涂片比较,液基细胞学制片的诊断时间更短。结论液基细胞学技术应用于FNAC检查中,在细胞形态、染色效果、染色优良率、标本阳性检出率及诊断一致率方面均具有明显优势,有助于提高甲状腺良恶性病变的诊断准确率,在病理诊断中具有重要应用价值。
Objective To compare the application value of traditional manual smear and liquid-based cytology technique in fine needle aspiration cytology(FNAC)of thyroid.Methods From January 2024 to June 2025,80 specimens of thyroid nodules received FNAC in Guigang Orthopedic Hospital of Integrated Traditional Chinese and Western Medicine were randomly selected and treated with traditional manual smear and liquid-based cytology technique respectively.The differences between the two methods in cell morphology,staining effect,excellent and good staining rate,positive detection rate and diagnostic consistency rate were compared.Results The cell morphology and staining effect of the liquid-based cytology preparation were better than those of the traditional manual smear.The excellent and good rate of staining of the liquid-based cytology preparation was 88.75%,which was higher than 70.00%of the traditional manual smear,and the difference was statistically significant(P<0.05).The positive detection rate of the liquid-based cytology preparation specimens was 31.25%,which was higher than 23.75%of the traditional manual smears,but the difference was not statistically significant(χ^(2)=1.13,P=0.29).The diagnostic consistency rate of the liquid-based cytology preparation was 92.59%,which was higher than 70.37%of the traditional manual smear,and the difference was statistically significant(χ^(2)=4.42,P=0.04).Compared with the traditional manual smears,the liquid-based cytology preparation had a shorter diagnosis time.Conclusion The liquid-based cytology technique applied in FNAC examination has obvious advantages in cell morphology,staining effect,excellent and good rate of staining,positive detection rate of specimens and diagnostic consistency rate,which is helpful to improve the diagnostic accuracy of benign and malignant thyroid lesions,and has important application value in pathological diagnosis.
