摘要
目的探究隐形矫治器在青少年Ⅱ类错[牙合]畸形矫治推磨牙向后中的应用效果。方法选取滨州医学院附属医院儿童牙病科2022年3月到2024年3月收治的50例安氏Ⅱ类错[牙合]畸形患儿,按照治疗方式不同分为对照组(n=25)和观察组(n=25)。对照组选择口外弓进行矫治,观察组使用加带矩形固位附件的隐适美隐形矫治器治疗。比较两组的口腔相关指标、牙周健康状况、美观效果、生活质量及家属的满意度。结果相较于对照组,观察组的上中切牙凸距差、上中切牙倾角差更大,磨牙移位距离更小(P<0.05)。相较于治疗前,两组随访后的菌斑指数(PLI)、龈沟出血指数(SBI)、牙龈指数(GI)评分均升高,但观察组低于对照组(P<0.05)。相较于治疗前,两组随访后的正畸治疗需要指数的美观量表(IOTN-AC)、口腔健康影响程度量表(OHIP-14)评分均降低,且观察组低于对照组(P<0.05)。观察组家属的满意度为92.00%,较对照组的68.00%更高(P<0.05)。结论隐形矫治器在青少年Ⅱ类错[牙合]畸形矫治推磨牙向后中的应用效果显著,能够改善口腔相关指标,且对牙周健康影响较小,同时提高患儿的生活质量及家属的满意度,值得应用及推广。
Objective To explore the application effect of invisible appliance in the treatment of molar retroversion in adolescents with classⅡmalocclusion.Methods A total of 50 children with classⅡmalocclusion admitted in the pediatric dentistry department of Binzhou Medical University Affiliated Hospital from March 2022 to March 2024 were selected and divided into control group(n=25)and observation group(n=25)according to different treatment methods.The control group was treated with extraoral arch,and the observation group was treated with invisible appliance with rectangular retention attachment.The oral related indicators,periodontal health status,aesthetic effect,quality of life and family satisfaction were compared between the two groups.Results Compared with the control group,the convex distance difference and the inclination angle difference of the upper central incisor in the observation group were larger,and the displacement distance of the molar was smaller(P<0.05).Compared with before treatment,the plaque index(PLI),sulcus bleeding index(SBI)and gingival index(GI)scores of the two groups increased after follow-up,but those of the observation group were lower than the control group(P<0.05).Compared with before treatment,the scores of Self-Rated Aesthetic Component of the Index of Orthodontic Treatment Need(IOTN-AC)and Oral Health Impact Profile(OHIP-14)in the two groups decreased after follow-up,and those of observation group were lower than the control group(P<0.05).The satisfaction of family members in the observation group was 92.00%,which was higher than 68.00%in the control group(P<0.05).Conclusion The invisible appliance applied in the treatment of molar retroversion in adolescents classⅡmalocclusion has significant effect.It can improve the oral related indicators,has little effect on periodontal health,and enhance the quality of life of children and the satisfaction of their families,which is worthy of application and promotion.
