摘要
目的观察白蛋白结合型紫杉醇联合替雷利珠单抗在复发性广泛期小细胞肺癌(ES-SCLC)二线治疗中的疗效。方法回顾性分析2021年3月至2023年10月于广州市胸科医院肿瘤科住院治疗的12例ES-SCLC患者。二线治疗接受白蛋白结合型紫杉醇(200 mg/m2,每3周1次)联合替雷利珠单抗(200 mg,每3周1次)。评估患者治疗前、后的血清肿瘤标志物指标、临床疗效及安全性。结果患者治疗后的神经元特异性烯醇化酶(NSE)、细胞角蛋白19片段(CYFRA21-1)及鳞状上皮细胞癌抗原(SCC-Ag)水平较治疗前明显下降,差异具有统计学意义(P<0.05)。12例患者中,无完全缓解(CR)患者,5例部分缓解(PR),4例疾病稳定(SD),3例疾病进展(PD)。客观缓解率(ORR)为41.67%,疾病控制率(DCR)为75.00%。患者的中位无进展生存期(PFS)为3.1个月,中位生存期(OS)为10.0个月。安全性方面,3级以上不良反应的发生率以骨髓抑制(白细胞减低、粒细胞减低、贫血、血小板减低)和乏力为主。无治疗导致死亡的病例。结论白蛋白结合型紫杉醇与替雷利珠单抗联合治疗可能成为复发性ES-SCLC二线治疗的可选择方案之一。
Objective To observe the efficacy of albumin-bound paclitaxel combined with tirelizumab in the second-line treatment of recurrent extensive-stage small cell lung cancer(ES-SCLC).Methods A retrospective analysis was performed on 12 patients with ES-SCLC who were hospitalized in the oncology department of Guangzhou Chest Hospital from March 2021 to October 2023.The second-line treatment received albumin-bound paclitaxel(200 mg/m2,once every 3 weeks)combined with tireilizumab(200 mg,once every 3 weeks).The serum tumor marker indexes,clinical efficacy and safety of patients before and after treatment were evaluated.Results The levels of neuron-specific enolase(NSE),cytokeratin 19 fragment(CYFRA21-1)and squamous epithelial cell carcinoma antigen(SCC-Ag)in patients after treatment were significantly lower than those before treatment,and the differences were statistically significant(P<0.05).Among the 12 patients,there were no complete response(CR),5 cases partial response(PR),4 cases stable disease(SD),and 3 cases progressive disease(PD).The objective response rate(ORR)was 41.67%,and the disease control rate(DCR)was 75.00%.The median progression-free survival(PFS)was 3.1 months,and the median overall survival(OS)was 10.0 months.In terms of safety,the incidence of adverse reactions above grade 3 was mainly bone marrow suppression(leukopenia,granulocytopenia,anemia,thrombocytopenia)and fatigue.There were no deaths caused by treatment.Conclusion The combination of albumin-bound paclitaxel and tirelizumab may be one of the options for second-line treatment of recurrent ES-SCLC.
