摘要
本文以社区慢病管理为切入点,阐述了基层社区卫生管理模式的现状、存在问题及创新探索方向。当前基层社区仍存在资源配置不均衡、居民参与度不高、信息化建设不完善等问题。本文提出整合资源构建新型医联体模式、强化居民健康素养教育、深化信息化建设等创新策略,旨在优化基层社区卫生管理,提升社区居民健康素养,为基层卫生管理提供理论与实践参考。
This paper takes community chronic disease management as the entry point,expounding the current situation,existing problems and innovative exploration directions of the community health management model at the grassroots level.The current grassroots communities still face problems such as unbalanced resource allocation,low resident participation,and incomplete information construction.This article proposes innovative strategies such as integrating resources to build a new medical alliance model,strengthening health literacy education for residents,and deepening information construction,aiming to optimize community health management at the grassroots level,enhance the health literacy of community residents,and provide theoretical and practical references for grassroots health management.
