原发性高血压患者HRV、baPWV、炎症因子水平与血压昼夜节律变化关系分析

Relationship between HRV,baPWV and inflammatory factors and the circadian rhythm of blood pressure in patients with essential hypertension

摘要 目的分析原发性高血压患者心率变异性(HRV)臂踝脉搏波传导速度(baPWV)炎症因子水平与血压昼夜节律间的关系。方法回顾性分析病例资料选自邢台市中心医院于2022年1月至2024年1月收治的118例原发性高血压患者,根据夜间血压下降率分组,将43例非杓型高血压纳入A组,将75例杓型高血压纳入B组,比较两组血压昼夜节律、HRV参数、baPWV及炎症因子水平变化;并分析患者夜间血压下降率与HRV各参数、baPWV、白细胞介素-6(IL-6)肿瘤坏死因子-α(TNF-α)水平间的关系。结果与B组相比,A组24h平均收缩压(24hSBP)、24h平均舒张压(24hDBP)、白昼平均收缩压(dSBP)、白昼平均舒张压(dDBP)夜间平均收缩压(nSBP)夜间平均舒张压(nDBP)、baPWV IL-6、TNF-α水平更高(P<0.05),A组的总体标准差(SDNN)低频功率(LF)、LF/HF值更低(P<0.05)。两组的均值标准差(SDANN)、差值均方根(rMSSD)、差值>50ms百分比(PNN50)高频功率(HF)间无显著差异(P>0.05)。经Pearson相关性分析,患者夜间血压下降率与baPWV、IL-6、TNF-α水平呈负相关(P<0.05),与SDNN、LF、LF/HF呈正相关(P<0.05)。结论原发性高血压患者血压昼夜节律与HRV、baPWV、炎症因子这些影响心血管事件相关因子间具有明显的相关性。 Objective To analyze the relationship of heart rate variability(HRV),brachial-ankle pulse wave velocity(baPWV)and inflammatory factors with blood pressure circadian rhythm in patients with essential hypertension.Methods A retrospective analysis was conducted on the clinical data of 118 patients with essential hypertension admitted to our hospital from January 2022 to January 2024.According to the nighttime blood pressure decline rate,43 patients with non-dipper hypertension were assigned to group A and 75 patients with dipper hypertension to group B.Blood pressure circadian rhythm,HRV parameters,baPWV,and inflammatory factor levels were compared between the two groups.The relationships between nighttime blood pressure decline rate and HRV parameters,baPWV,IL-6,and TNF-αlevels were also analyzed.Results Compared with group B,group A had higher 24-hour mean systolic blood pressure(24h SBP),24-hour mean diastolic blood pressure(24h DBP),daytime mean systolic blood pressure(dSBP),daytime mean diastolic blood pressure(dDBP),nighttime mean systolic blood pressure(nSBP),nighttime mean diastolic blood pressure(nDBP)],baPWV,interleukin-6 and tumor necrosis factor(TNF)-α(P<0.05),while the standard deviation of normal RR intervals(SDNN),low frequency power(LF)and LF/HF were lower(P<0.05).There were no significant differences between the two groups in standard deviation of 5-minute mean RR intervals(SDANN),root mean square of difference(rMSSD),percentage of difference>50ms(PNN50)and high frequency power(HF)(P>0.05).Pearson correlation analysis showed that nighttime blood pressure decline rate was negatively correlated with baPWV,IL-6,and TNF-α(P<0.05),and positively correlated with SDNN,LF,and LF/HF(P<0.05).Conclusion The circadian rhythm of blood pressure in patients with essential hypertension is signifi cantly correlated with HRV,baPWV,and infammatory factors,which are associated with cardiovascular events.
