摘要
目的:本研究基于德尔菲法,构建一套科学合理的街镇综合医院转型为中医类别医院的评价指标体系,发挥中医特色优势。方法:通过查阅综合医院转型为中医类别医院的相关文献及标准,结合专家访谈,初步形成候选指标;定性研究构建评价指标体系框架,运用德尔菲法对指标体系进行修订和完善,同时结合综合医院实际发展情况,形成评价指标池;制作调查问卷,采用德尔菲法及层次分析法(AHP)确定最终的评价指标体系,并计算变异系数及权重,建立综合医院转型为中医类别医院的评价指标体系。结果:构建的评价体系包括2级指标。一级指标共11项,具体为:中医药卫生人力、患者满意度、医院服务提供量、中医人才培养与文化建设、科室及设备设置、中医药服务项目开设情况、中医药特色服务提供情况、中医优势病种及诊疗规范、中医药收入及支出情况、中医药科研成果、中医(中西医)临床路径开展情况等;二级指标共45项,且各指标权重分布合理。结论:德尔菲法构建的评价体系囊括了街镇综合医院转型为中医类别医院中具有代表性的关键指标,不仅为评估医院转型提供科学工具,也为中医类别医院制定高质量发展政策提供了参考依据,对提升综合医院的转型效率及推动中医药特色优势的发挥具有重要意义。
Objective:Based on the Delphi method,this study aims to establish a scientific and reasonable evaluation index system for the transformation of community hospitals into Chinese medicine-specialized hospitals,to leverage the characteristics and advantages of Chinese medicine.Methods:Candidate indicators were preliminarily formed by reviewing relevant literature and standards on the transformation of community hospitals into Chinese medicine-specialized hospitals,and combined with expert interviews.Qualitative research was used to construct the framework of the evaluation index system,and the Delphi method was applied to revise and improve the system.Considering the actual development of community hospitals,an indicator pool was established.A questionnaire was developed,and the Delphi method and Analytic Hierarchy Process(AHP)were used to determine the final evaluation index system,calculate the coefficient of variation and weights,and establish the evaluation index system for the transformation of community hospitals into Chinese medicine-specialized hospitals.Results:The constructed evaluation system includes two levels of indicators.The first level consists of 11 items:traditional Chinese medicine health human resourses,patient satisfaction,hospital service volume,Chinese medicine talent cultivation and cultural development,department and equipment setup,Chinese medicine service offerings,provision of Chinese medicine-characteristic services,Chinese medicine-advantaged diseases and treatment protocols,Chinese medicine-related income and expenditure,Chinese medicine research achievements,and implementation of Chinese medicine(integrated Chinese and western medicine)clinical pathways.The second level includes 45 items,with reasonable weight distribution for each indicator.Conclusion:The evaluation system constructed using the Delphi method encompasses representative key indicators for the transformation of community hospitals into Chinese medicine-specialized hospitals.It not only provides a scientific tool for assessing hospital transformation but also serves as a reference for formulating high-quality development policies for Chinese medicine-specialized hospitals.This system is of significant importance for improving the efficiency of community hospital transformation and promoting the advantages of Chinese medicine.
