摘要
目的评估泪囊鼻腔吻合术(Dacryocystorhinostomy,DCR)结合改良泪道引流管植入术在治疗慢性泪囊炎方面的临床效果与价值。方法纳入50例慢性泪囊炎患者,随机分为联合组(DCR+改良引流管植入,n=25)和单一组(单纯DCR,n=25),评估两组术后1周及术后3个月的泪道通畅率、拔管后(术后3个月)的症状缓解情况;对联合组术后1周及术后3个月的泪道通畅率进行比较。结果术后1周联合组实现泪道不通畅率清零(0vs20.0%,P<0.05),至术后3个月完全通畅率突破性提升至92.0%(较单一组56.0%提高36.0%,P<0.05),且联合组自身通畅率较术后1周显著增长28.0%。结论结果表明采用泪囊鼻腔吻合术联合改良泪道引流管植入术治疗慢性泪囊炎,可显著提高泪道重建成功率,并有效缓解患者临床症状,具有重要的临床推广价值。
Objective To evaluate the clinical efficacy and value of dacryocystorhinostomy(DCR)combined with modified lacrimal drainage tube implantation in the treatment of chronic dacryocystitis.Methods Fifty patients with chronic dacryocystitis were enrolled and randomly assigned to either the combination group(DCR+modified tube implantation,n=25)or the single-procedure group(DCR alone,n=25).The lacrimal duct patency rate at 1 week and 3 months postoperatively,and symptom relief after tube removal(at 3 months postoperatively)were assessed in both groups.The patency rate at 1 week and 3 months postoperatively was also compared within the combination group.Results At 1 week postoperatively,the combination group achieved a 0 rate of lacrimal duct obstruction(0vs.20.0%,P<0.05).By 3 months postoperatively,the complete patency rate in the combination group significantly increased to 92.0%(36.0%higher than the 56.0%in the single-procedure group,P<0.05).Furthermore,the patency rate within the combination group showed a significant increase of 28.0%compared to the 1-week postoperative level.Conclusion The results demonstrated that dacryocystorhinostomy(DCR)combined with modified lacrimal drainage tube implantation significantly improved the success rate of lacrimal duct reconstruction and effectively alleviated clinical symptoms in patients with chronic dacryocystitis,highlighting its significant clinical value for widespread adoption.
