摘要
目的探讨基于授权理论的自我管理模式对心房扑动患者射频消融术后自我管理能力及心房颤动的防控作用。方法选取2020年3月至2024年1月郑州大学第一附属医院收治的134例心房扑动射频消融术后患者作为研究对象,按随机数表法分为对照组和研究组务67例。对照组患者给予常规护理干预,研究组患者在对照组护理的基础上给予基于授权理论的自我管理模式,两组均干预3个月。比较两组患者干预前、干预3个月后(干预后)的自我管理能力[成年人健康自我管理能力测评量表(AHSMSRS)]、负性情绪[焦虑自评量表(SAS)、抑郁自评量表(SDS)]、生活质量[生活质量综合评定问卷-74(GQOLI-74)]及干预后随访3个月的并发症发生率。结果干预后研究组患者AHSMSRS的自我管理认知、行为、环境评分及总分分别为(60.36±2.03)分、(61.25±1.17)分、(45.17±1.51)分、(166.78±5.63)分,明显高于对照组的(52.79±2.38)分、(50.42±2.59)分、(37.22±2.39)分、(140.43±8.95)分,差异均有统计学意义(P<0.05);干预后研究组患者的SAS、SDS评分分别为(38.17±5.18)分、(35.09±4.79)分,明显低于对照组的(50.36±6.55)分、(52.36±7.16)分,差异均有统计学意义(P<0.05);干预后研究组患者GQOLI-74的社会、躯体、心理功能、物质生活状态评分及总分分别为(95.33±1.21)分、(92.38±1.52)分、(94.22±1.15)分、(96.78±1.07)分、(378.71±5.63)分,明显高于对照组的(90.17±1.39)分、(87.62±2.03)分、(87.96±2.15)分、(92.24±1.63)分、(357.99±7.89)分,差异均有统计学意义(P<0.05);干预后随访3个月,研究组患者的并发生总发生率为1.54%,明显低于对照组的20.63%,差异有统计意义(P<0.05)。结论基于授权理论的自我管理模式可提升心房扑动患者射频消融术后自我管理能力,减轻患者负性情绪,预防心房颤动等并发症发生,提升患者生活质量。
Objective To explore the effect of an empowerment theory-based self-management model on self-management ability and the prevention and control of atrial fibrillation in patients with atrial flutter after radiofrequency ablation.Methods A total of 134 patients who underwent radiofrequency ablation for atrial flutter at the First Affiliated Hospital of Zhengzhou University from March 2020 to January 2024 were selected as research subjects.According to the random number table method,these patients were divided into a control group and a study group,with 67 cases in each group.Patients in the control group received routine nursing care,and those in the study group were given an empowerment theory-based self-management model approach in addition to the standard care provided to the control group.Both groups were intervened for 3 months.Comparisons were made between the two groups for self-management ability(Adult Health Self-Management Skill Rating Scale[AHSMSRS]),negative emotions(Self-rating Anxiety Scale[SAS],Self-rating Depression Scale[SDS]),and quality of life(Generic Quality of Life Inventory-74[GQOLI-74])at pre-intervention and at 3-months post-intervention,as well as for complication incidence observed during the 3-month post-intervention follow-up.Results After intervention,the scores of self-management cognition,behavior,environment,and total score of AHSMSRS in the study group were(60.36±2.03)points,(61.25±1.17)points,(45.17±1.51)points,and(166.78±5.63)points,respectively,significantly higher than corresponding(52.79±2.38)points,(50.42±2.59)points,(37.22±2.39)points,and(140.43±8.95)points in the control group(all P<0.05).After intervention,the SAS and SDS scores of the study group were(38.17±5.18)points and(35.09±4.79)points,respectively,which were significantly lower than corresponding(50.36±6.55)points and(52.36±7.16)points in the control group(both P<0.05).After intervention,the scores of social function,physical function,psychological function,material living status,and total score of GQOLI-74 in the study group were(95.33±1.21)points,(92.38±1.52)points,(94.22±1.15)points,(96.78±1.07)points,and(378.71±5.63)points,respectively,significantly higher than corresponding(90.17±1.39)points,(87.62±2.03)points,(87.96±2.15)points,(92.24±1.63)points,and(357.99±7.89)points in the control group(all P<0.05).During the 3-month follow-up after the intervention,the total incidence of complications in the study group was 1.54%,significantly lower than 20.63%in the control group(P<0.05).Conclusion The empowerment theory-based self-management model can improve self-management ability,reduce negative emotions,prevent atrial fibrillation and other complications,and improve the quality of life in patients with atrial flutter after radiofrequency ablation.
