摘要
目的分析微型种植体支抗矫治安氏Ⅱ^(1)分类错[牙合]的临床效果。方法将200例安氏Ⅱ^(1)分类错[牙合]患者随机分为研究组与对照组,每组100例。两组均采用直丝弓矫治器开展正畸治疗,研究组采用微型种植体植入作为支抗手段治疗,对照组使用传统口内支抗Nance弓辅助治疗。对比两组头影测量颌骨相关指标[上齿槽座角(SNA)、下齿槽座角(SNB)、上下齿槽座角(ANB)、下颌平面与前颅底平面夹角(MP-SN)、颌平面与前颅底平面夹角(OP-SN)]、软组织相关指标[上唇突度、下唇突度、上唇突点至E线垂直距离(Ulip-E)、下唇突点至E线垂直距离(Llip-E)、颏前点及上下唇突点切线与眼耳平面夹角(Z角)]、牙齿相关指标[上中切牙长轴与前颅底平面的后下夹角(U1-SN)、上中切牙切缘至鼻根点(N)与上牙槽座点(A)的连线(NA连线)垂直距离(U1-NA)、上中切牙到颏前点的垂直距离(U1-APo)、下中切牙长轴与鼻根点下齿槽座点连线的距离(L1-NB)]及矫治治疗时间。结果治疗后,同对照组相比,研究组的SNA、SNB、ANB、MP-SN、OP-SN、上唇突度、下唇突度、Ulip-E、Llip-E、Z角、U1-SN、U1-NA、U1-APo、L1-NB更优(P<0.05)。研究组矫治治疗时间(2.02±0.12)年相比对照组的(2.49±0.08)年更短(P<0.05)。结论同传统口内支抗Nance弓辅助治疗相比,微型种植体支抗矫治对安氏Ⅱ^(1)分类错[牙合]的矫治效果更突出,能够提升正畸治疗质量,帮助患者获得牙齿功能与美观度兼具的理想治疗效果,同时可以优化咬合功能,塑造面部轮廓,还可通过缩短矫治时长,减轻患者身心压力,具有广泛应用价值。
Objective To analyze the clinical effect of micro-implant anchorage in the treatment of Angle's classⅡ^(1) malocclusion.Methods A total of 200 patients with Angle's classⅡ^(1) malocclusion were selected,and the patients were randomly divided into study group and control group,each comprising 100 cases.All of whom were treated with straight wire appliance.Among them,the study group used micro-implant implantation as anchorage means,while the control group used traditional anchorage Nance arch as adjuvant therapy.The following parameters of the two groups were compared:jawbone-related cephalometric indices[sella-nasion-subspinale angle(SNA),sella-nasion-supramental angle(SNB),subspinale-nasion-supramental angle(ANB),mandible plane and sella-nasion line angle(MP-SN),occlusal plane and sella-nasion plane angle(OP-SN)],soft tissue-related indices[upper lip protrusion,lower lip protrusion,distance from upper lip to E-line(Ulip-E),distance from lower lip to E-line(Llip-E),Z-angle],dental-related indicators[upper incisor-sella-nasion angle(U1-SN),distance from upper incisor to nasion-subspinale line(U1-NA),distance from upper incisor to A-pogonion(U1-APo),distance from lower incisor to nasion-supramental line(L1-NB)],and correction time.Results Compared with the control group,the improvements of SNA,SNB,ANB,MP-SN,OP-SN,upper lip protrusion,lower lip protrusion,Ulip-E,Llip-E,Z angle,U1-SN,U1-NA,U1-APo and L1-NB in the study group were more significant after treatment(P<0.05).The correction time of the study group was(2.02±0.12)years,which was shorter than the control group's(2.49±0.08)years(P<0.05).Conclusion Compared with the traditional auxiliary treatment of anchorage Nance arch,micro-implant anchorage is more effective in the treatment of Angle's classⅡ^(1) malocclusion,which can improve the quality of orthodontic treatment and help patients obtain the ideal treatment effect with both dental function and aesthetics.It can optimize the occlusal function,shape the facial contour,and reduce the physical and mental stress of patients by shortening the correction time,which has wide application value.
