摘要
小儿咳嗽变异性哮喘(CVA)以持续性干咳及气道高反应性为特征,发病率逐年上升。现代医学依赖吸入性糖皮质激素(ICS)及支气管舒张剂治疗该病,但长期用药易引起生长迟滞、免疫抑制等不良反应。基于中医“肺与大肠相表里”理论,研究提出肺-肠轴功能失调是CVA迁延难愈的核心病机:肺失宣肃,可致大肠传导失司,肠腑壅滞反加重肺气上逆,形成恶性循环,据此构建了中西医协同的“调肺通腑”治疗策略。中西医协同模式不仅减少了激素用量及用药不良反应,更通过多靶点干预,实现肺肠同治,为小儿CVA的个体化诊疗提供了理论支持。
Pediatric cough variant asthma(CVA)is characterized by persistent dry cough and airway hyperresponsiveness,and its incidence is increasing annually.Modern medicine relies on inhaled corticosteroids(ICS)and bronchodilators for treatment,but long-term use can easily lead to adverse reactions such as growth retardation and immunosuppression.Based on the traditional Chinese medicine theory of the"interior-exterior relationship between the lung and the large intestine,"this study proposes that dysfunction of the lung-intestine axis is the core pathogenesis underlying the persistent and refractory nature of CVA:impaired dispersion and descent of lung qi can lead to dysfunction in the conveyance function of the large intestine,and stagnation in the intestinal tract in turn aggravates the counterflow of lung qi,forming a vicious cycle.Accordingly,an integrated Chinese-Western medicine strategy of"regulating the lung and unblocking the fu-viscera"has been established.This integrated approach not only reduces the dosage of hormones and the associated adverse drug reactions but also achieves simultaneous treatment of the lung and intestine through multi-target interventions,providing theoretical support for the personalized diagnosis and treatment of pediatric CVA.
