摘要
乳腺癌是严重威胁女性健康的恶性肿瘤,其高侵袭性与肿瘤血管化密切相关,寻求有效的治疗策略成为临床关键。本研究基于中医理论进行深入探讨,提出正虚毒结是乳腺癌的核心病机,其中脏腑失调、正气虚弱是疾病发生发展的根本,“郁痰瘀毒互结”则是病情进展的关键因素。进一步结合现代医学对肿瘤血管化的研究,剖析肿瘤血管生成的病理特征与中医学中“络脉迂曲”的相似性,本研究诠释了“瘀毒损络”病机与肿瘤血管内皮生长、血供及转移倾向的内在联系。在此基础上,本研究提出疏肝解毒法调控肿瘤血管化的两大作用靶点,通过疏肝调畅气机阻断血管生成的始动因素,从源头抑制促血管生成因子的异常分泌;通过解毒化瘀通络直接抑制血管生成,改善肿瘤微环境。研究表明,疏肝解毒法从中医病机与现代医学机制的双重维度为乳腺癌的辨治提供了新的理论依据与潜在治疗方向,对拓展中医药在乳腺癌治疗中的应用具有重要意义。
Breast cancer is a malignant tumor that seriously threatens women’s health,and its high invasiveness is closely related to tumor vascularization.Seeking effective treatment strategies for the disease has become a key point in clinical practice.In this paper,based on the in-depth discussion of relevant traditional Chinese medicine theory,it is proposed that the core pathogenesis of breast cancer is deficient vital energy and toxin accumulation,the disorder of internal organs and the weakness of vital energy are the fundamental factors of the disease’s occurrence and development,while“the mutual accumulation of pathogenic toxins such as depression,phlegm,and blood”is the key factor of the progression of the disease.Further combined with the research on tumor vascularization in modern medicine,the similarity between the pathological characteristics of tumor angiogenesis and the“tortuous meridians”in traditional Chinese medicine was analyzed,and the intrinsic connection between the pathogenesis of“blood stasis and toxin damaging meridians”and the growth of tumor vascular endothelium,blood supply,and metastasis tendency was interpreted.On this basis,two major targets for regulating tumor vascularization by the method of soothing the liver and detoxifying were proposed as followed:By soothing the liver and regulating the smooth flow of Qi(气),the initiating factors of angiogenesis were blocked,and the abnormal secretion of pro-angiogenic factors was inhibited from the source;By detoxifying,removing blood stasis and unblocking meridians,it directly inhibited angiogenesis and ameliorated the tumor microenvironment.Studies have shown that the method of soothing the liver and detoxifying provides new theoretical basis and potential therapeutic directions for the diagnosis and treatment of breast cancer from the dual dimensions of traditional Chinese medicine pathogenesis and modern medical mechanisms,which is of great significance for expanding the application of traditional Chinese medicine in the treatment of breast cancer.
