基于静息态脑电图分析轻度认知障碍患者δ频段脑区神经振荡异常和动态脑网络异常连接模式

Neural Oscillation Abnormalities and Abnormal Dynamic Network Connectivity in δ Frequency Band in Patients with Mild Cognitive Impairment Based on Resting-State Electroencephalogram

摘要 目的基于静息态脑电图(rsEEG)分析轻度认知障碍(MCI)患者δ频段脑区神经振荡异常以及动态脑网络异常连接模式。方法2022年11月—2023年5月在成都市青羊区民安社区选择41例老年人作为研究对象,其中21例为MCI组,20例为健康对照组(对照组)。采用EEG采集系统采集受试者5 min的静息态EEG数据。分析δ频段内各电极的功率谱密度(PSD);采用定向传递函数(DTF)构建静息态的动态因果脑网络,同时分析差异具有统计学意义的脑连接以及其与蒙特利尔认知评估(MoCA)评分的相关性。结果①PSD:在δ频段,对照组在前额叶(F7)1个电极、顶叶(CP6)1个电极以及枕叶(PO3、PO7)2个电极表现出更强的δ振荡(P<0.05)。MCI组在前额叶(F8、FP2、AF3、Fz、FC1、FC2、FC5)7个电极、顶叶(CP1、CP2、CP5、P3、P4、P7、P8、Pz)8个电极、颞叶(T7)1个电极以及中央区(C4、Cz)2个电极表现出更强的δ振荡(P<0.05)。②静息态非时变动态网络:在δ频段,对照组有明显增强的前额叶(AF3)流向前额叶(AF4)的脑连接(P<0.05)。MCI组有明显增强的自上而下的前额叶(Fz、F8)流向顶叶(P3)的脑连接,自下而上的顶叶(CP5)流向前额叶(FP2、F3)的脑连接;顶叶(P3)流向中央区(C3)以及自下而上的枕叶(PO3)流向前额叶(F7)、中央区(C4)和顶叶(CP5)的脑连接(P<0.05)。静息态状态下,与MoCA评分显著相关的增强脑连接包括前额叶和顶叶交互脑连接(r=-0.37,P=0.012;r=-0.41,P=0.007),以及自下而上的枕叶到中央区以及顶叶长程连接(r=-0.34,P=0.030;r=-0.37,P=0.010)。结论MCI患者表现出多脑区多电极的δ振荡增强,主要分布在额叶以及顶叶。在δ频段,静息态脑网络连接明显异常,主要表现为前额叶和顶叶的相互连接以及自下而上的枕叶到中央区以及顶叶的脑连接明显增强。 Objective To analyze the abnormalities in neural oscillations within brain regions as well as the abnormal connectivity patterns of dynamic brain networks in the delta frequency band of patients with mild cognitive impairment(MCI)based on resting-state electroencephalogram(rsEEG).Methods A total of 41 elderly individuals were selected from Min'an Community,Qingyang District,Chengdu City from November 2022 to May 2023 as the study subjects,including 21 cases in the MCI group,and 20 cases in the healthy control group(control group).An EEG device was used to collect 5-minute resting-state EEG data.The power spectral density(PSD)was analyzed for each electrode within the δ frequency band;the directed transfer function(DTF)was used to construct dynamic causal brain networks in the resting state,and the brain connectivity with statistically significant differences and their correlation with Montreal Cognitive Assessment(MoCA)score were analyzed.Results(1)PSD:in theδfrequency band,stronger δ oscillations were observed at one electrode in the prefrontal lobe(F7),one in the parietal lobe(CP6),and two in the occipital lobe(PO3,PO7)in the control group(P<0.05).δ oscillations were stronger at seven electrodes in the prefrontal lobe(F8,FP2,AF3,Fz,FC1,FC2,FC5),eight in the parietal lobe(CP1,CP2,CP5,P3,P4,P7,P8,Pz),one in the temporal lobe(T7),and two in the central region(C4,Cz)in the MCI group(P<0.05).(2)Resting-state time-invariant dynamic networks:in the δ frequency band,significantly enhanced brain connectivity was observed from the prefrontal lobe(AF3)to the prefrontal lobe(AF4)in the control group(P<0.05).The MCI group showed significantly enhanced top-down connectivity from prefrontal lobe(Fz,F8)to parietal(P3)lobe,bottom-up connectivity from parietal lobe(CP5)to prefrontal(FP2,F3)lobe,and connectivity from parietal lobe(P3)to central(C3)region,as well as bottom-up connectivity from occipital lobe(PO3)to prefrontal lobe(F7),central region(C4),and parietal lobe(CP5)(P<0.05).In the resting state,the enhanced brain connections significantly correlated with the MoCA score included the interactive brain connections between the prefrontal and parietal lobes(r=-0.37,P=0.012;r=-0.41,P=0.007),and the bottom-up long-range connections from the occipital lobe to the central region and parietal lobe(r=-0.34,P=0.030;r=-0.37,P=0.010).Conclusion MCI patients showed enhanced δ oscillations across multiple electrodes in various brain regions,primarily in the frontal and parietal lobes.In the δ frequency band,resting-state brain network connectivity exhibited significant abnormalities,mainly manifesting as enhanced mutual connectivity between the prefrontal and parietal lobes,and bottom-up connectivity from the occipital lobe to the central region and parietal lobe.
