摘要
血液凝固性增加是指血液中血小板和凝血因子增多,或纤维蛋白溶解系统活性降低,会导致血液的高凝状态,有相关实验室指标可作为诊断依据。血液高凝状态可见于多种疾病中,并且这些疾病在临床中非常多见,如广泛转移的晚期恶性肿瘤、冠状动脉粥样硬化性心脏病、糖尿病、脑梗死等。血液高凝状态是血栓形成的重要因素,也对疾病的进展有一定影响,故及时对其进行干预是有意义的。近年来,有学者对针灸治疗改善高凝状态做了一些研究,已发表的文献内容包括开展动物实验、临床试验进行机制探究,以及临床经验分享等。在查阅相关文献后对其进行分类整理,然后拟从针灸治疗对高凝状态的影响、针灸疗法对血栓形成的干预、针灸治疗的活血作用相关研究、具有活血作用的相关穴位的研究、选择针刺穴位以活血化瘀的治疗思路这几个方面进行综述,从针灸疗法干预高凝状态的应用前景、针灸疗法干预高凝状态研究现状的不足之处等方面对针灸疗法改善高凝状态的现状进行讨论,并提出挖掘更高质量的循证依据,制定标准化和国际化的针灸方案,找出适合的疗效评价指标以客观评价针刺的效果,对比针刺治疗与双抗、抗凝药物疗效的研究等进一步研究方向。
Increased blood coagulability refers to an increase in platelets and coagulation factors in the blood,or a decrease in the activity of the fibrinolytic system,which can lead to a hypercoagulable state of blood.Relevant laboratory indicators can be used as diagnostic basis.Blood hypercoagulability can be seen in a variety of diseases,and these diseases are very common in clinical practice,such as extensive metastatic advanced malignant tumors,coronary atherosclerotic heart disease,diabetes mellitus,and cerebral infarction.Blood hypercoagulability is an important factor in thrombosis,and also has a certain impact on the progression of the disease.Therefore,timely intervention on it is meaningful.In recent years,some scholars have conducted research on the amelioration of hypercoagulability by acupuncture intervention,and the published literature includes conducting animal experiments or clinical trials to explore the mechanism,clinical experience sharing,etc.In this study,after reviewing relevant literature and classifying and sorting out it,it is intended to elaborate on several aspects:the effects of acupuncture treatment on hypercoagulability,the intervention of acupuncture therapy on thrombosis,the related research on the blood-invigorating effect of acupuncture treatment,the research on relevant acupuncture points with blood-activating effects,and the therapeutic ideas of selecting acupuncture points to promote blood circulation and remove blood stasis.Additionally,the current situation of acupuncture therapy intervention on hypercoagulable state is discussed from several aspects,such as the application prospect of acupuncture therapy in the intervention on hypercoagulable state,and the shortcomings of the research status of acupuncture therapy in the intervention on hypercoagulable state.It is suggested to mine higher quality evidence-based medical evidence,formulate standardized and internationalized acupuncture programs,and find suitable therapeutic efficacy evaluation indicators to evaluate the effect of acupuncture objectively.Research on comparing the efficacy of acupuncture treatment with dual anti-platelet therapy and anticoagulant drugs may be the direction of further research.
